Since 1971, Hampton-Newport News Community Services Board has provided a comprehensive continuum of high-quality, essential, and community-based services to citizens of the Peninsula who are affected by mental illness, developmental disabilities, and substance use disorders.

Assertive Community Treatment, or ACT is a team of medical, behavioral health, and rehabilitative professionals who use a team approach to meet the needs of an individual with severe and persistent mental illness. Admission criteria include severe and persistent mental illness, predominately Schizophrenia, and high service needs in order to maintain stability.

Incumbent ensures continuity of directly provided and contractual services to agency's mentally ill, with possible co-occurring intellectual developmental disabilities and substance abusing client populations. The Service and Housing Coordinator will help the individuals identify housing needs and assist them in applying for housing opportunities, including development of a housing plan. Service and Housing Coordinator will manage a caseload and assist clients with their basic physical, financial, social and personal needs. Service coordination services for the client include outreach, psychological and need assessments, treatment planning and service provider linkage, monitoring, advocacy, counseling and crisis intervention, daily living support, medication supervision, and similar functions.

Requirements: Bachelors degree in Human Services (Social Work, Psychology or Counseling) and a QMHP-A or able to register with the Virginia Board of Counseling to practice as a QMHP-Trainee within the first 30 days of employment and one (1) year experience in case management or service coordination services, including housing resources for adults with mental health, developmental disabilities, substance use and co-occurring disorders.

The selected candidate must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.
