We achieve our mission by assessing individuals’ needs and strengths, linking them with community services and supports, and monitoring their response to treatment and the quality of care provided. Adult Mental Health Case Managers ensures continuity of directly provided and contractual services to agency's mental health, developmental disabilities and substance use disorder client populations. Adult Mental Health Case Managers assist clients with their basic physical, financial, social and personal needs. Case management services for the client include outreach, psychological and need assessments, treatment planning and service provider linkage, monitoring, advocacy, counseling and crisis intervention, daily living support, medication supervision, and similar functions.
