For frequently asked questions, please click here: https://www.catholiccharitiestrenton.org/careers/frequently-asked-questions/   Req Number DIR-23-00034   Location PH - Burlington County (Safe House)   Full-Time/Part-Time Full-Time   Number of Openings 1   Category Direct Care - Professionals / Paraprofessionals   Description JOB SUMMARY:      Responsible for the oversight of delivering case management/counseling services for clients and families.    ESSENTIAL FUNCTIONS:  Design, implement and monitor individual program plans for clients Assess client needs; investigate their eligibility for services Counsel and aids individuals and families requiring agency assistance Maintain client records and completes required documentation according to agency and accreditation standards.  This may include, but is not limited to data entry, progress notes, activity reports, time sheets and logs Coordinate client activities Assist clients with accessing other services (job training, counseling, housing, medical services, etc.) Network with other providers to generate community resources for participants May provide supportive counseling in individual and group settings May transport consumers to appointments, etc.   PROGRAM RESPONSIBILITIES: Continue with case manager responsibilities - including hotline coverage, group/individual case management, transportation, assessment and documentation Provide supervision and oversight to assigned case managers Assist with the review of files/hotlines as needed Participate in the on-call rotation and/or continue to cover one weekend shift per week Advocate as a supervisor with external agencies Assist with supervisory coverage when required  Monitor Safe House staff for accurate documentation in files and all forms Assist with training of newly hired employees/interns   OTHER DUTIES:  as assigned   Position Requirements MINIMUM QUALIFICATIONS:  Five years of human services experience OR relevant professional credential such as CPRP, CRC, CADC, OR Associates degree plus two years of experience OR Bachelor’s degree in human services field, rehabilitation or nursing. Two years of experience as a Safe House Case Manager preferred. LICENSES/CERTIFICATIONS NEEDED:  Valid driver’s license MANDATED PROGRAM REQUIREMENTS:  Providence House is a 24-hour residential crisis program, and as such requires staffing at all times. When accepting a position with this agency it is important to know that the regular shift at the time of hiring is subject to change during the course of your employment, and that at times of staff emergencies or special events you may be required to fill shifts other than (instead of) your regular shift.  This may include holidays.
