The Child and AdolescentSupport Coordinator ensures continuity of directly provided and contractual services to agencys individuals with intellectual and developmental disabilities. Support Coordinators assist individuals with their basic physical, financial, social and personal needs. Support Coordination services for the individuals include development of person centered plans, service provider linkage, monitoring, advocacy, counseling crisis intervention, daily living support, and enhancing community integration.

Requirements: Bachelors degree in Human Services (Social Work, Psychology,or Counseling) with one (1) year experience in support coordination services with the assigned population.
Valid Drivers License.
The selected candidate must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.
