Description

ProMedica Senior Care, formerly HCR ManorCare, provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy.

Directs and coordinates all aspects of non-nursing ancillary services such as social work, spiritual care, music therapy, massage therapy, volunteer services, bereavement services, occupational therapy, physical therapy. This position is responsible for the assessment and evaluation of patients and the supervision, instruction and evaluation of support service field staff who are engaged in the delivery of health care services. The director assures compliance with agency standards and state and federal laws. Assists in implementing policies and procedures and ensures all services meet the objectives of the organization.

Responsibilities

  • Oversees admission process; conducts on-site initial patient assessment and periodic reassessment and communicates with physician as needed.
  • Plans, coordinates and delivers care to patients and families as appropriate.
  • Delegates and assigns the patient/family to appropriate staff and team members, ensuring highest quality of care.
  • Visits patients and families as needed.
  • Supervises the staff responsible for ancillary services and provides assistance with development, implementation and evaluation of the respective programs.
  • Schedules, maintains and facilitates interdisciplinary team meetings for continual updating of patient care plans,exchange of information and problem solving.
  • Develops and promotes programs for continuing staff support, in-service and education.
  • Reviews quarterly audits of Volunteer and Bereavement programs ensuring compliance.
  • Conducts regular audits of support service documentation, ensuring compliance and individualized care plans and frequency compliance.
  • Implements and monitors compliance with rules and regulations of Federal and State regulatory agencies and accreditation requirements, and policies & procedures of agency.
  • Assures complete and accurate support service documentation in medical charts in compliance with all regulatory agencies. Assures peer reviews are complete and timely.
  • Participates in the interviewing, hiring and providing orientation and training for the staff.
  • Completes staff performance evaluations/counseling/implementing appropriate disciplinary action of employees.
  • Accepts other assignments deemed appropriate per Administrator.
  • Collaborates with service vender regarding appropriate cost-effective services and accurate billing.
  • Directs branch quality improvement programs.
  • Evaluates employees on-site as required per policy.
  • Participates in on-call coverage.
  • Monitors direct patient care costs and implements staff education to deliver cost effective, high quality and appropriate care to patients/families.
  • May serve on Agency Governing Board, QAPI, Safety Committee and IOP Committee.
 
Educational Requirements Bachelor's degree in social work, counseling, theology or related field required. May require licensure or certificate as required in the state in which they practice and be in good standing.  
Position Requirements

Minimum 3 years’ experience in providing medical, oncology, home health care or hospice experience.
Management or supervisory experience preferred.
