What You Will be Doing
Primary job responsibilities involve supervision of multiple Care Coordination Supervisors and teams as they complete needs assessment, service and resource linkage, and care coordination to support youth and their families and caregivers in the OhioRISE plan in achieving their health and outcome goals. The Care Coordinator Manager maybe both office and community-based and may perform some tasks from home. The Care Coordinator Manager will engage with individuals, parents/guardians, Boundless staff, community members, and other service and support providers via face-to-face engagement, telephone, video conferencing, and electronic communication. Day-to-day activities are varied based on the needs of the team members and communities. The Care Coordination Manager is responsible for ensuring the teams under their supervision are trained and following all required standards, guidelines, policies, and regulations. The Care Coordination Manager is accountable for ensuring the teams under their supervision meet all expected timelines and outcomes, including completion of assessments, outreach, documentation, and reporting.
Minimum Education/Qualifications:
- Master’s Degree in Social Work or related field of study.
- Three years of experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field, providing community-based services to children and youth and their family or caregivers.
- Experience in one or more of the following areas of expertise:
- Family systems
- Community systems and resources
- Case management
- Child and family counseling or therapy
- Child protection
- Child development
- Training in cultural competency or the ability to complete such within 90 days of hire.
- Three years of supervisory or management experience.
Required Licensure/Certification:
- Current Ohio mental health professional license including LISW, LPCC, LIMFT, PSY, LIMFT-S, LISW-S, or LPCC-S.
- Valid Ohio Driver’s License with Ohio Bureau of Motor Vehicles - No more than 5 points on driving record.
- Valid car insurance.
- Current High-Fidelity Wrap-Around and CANS Assessor training or the ability to complete such within 90 days of hire.
