Description

JOB SUMMARY:This position is responsible for the coordination and delivery of clinical behavioral health services designed to meet the wellness and recovery needs of clients within the EISS program. Depending on program objectives, these services may include resource development, assessments and counseling provided individually or in a group setting and may include the consumers family members.

ESSENTIAL FUNCTIONS

Intake:

Conduct initial and ongoing assessments involving the collection, analysis, and recording of significant bio-psychosocial information. Makes diagnoses

Clinical Services:

Provide individual, family and group counseling to consumers as needed. Provides psychoeducation for individuals and groups to reduce resistance to mental health treatment and enhance engagement. Maintains regular therapeutic contact with clients, including the provision of on-site services and support. Treatment plans for clients on medication prescribed by program staff will be done in consultation with the prescriber.

Documentation:

Enter reports and assessments into information technology system as indicated by the Program Director. Maintain accurate and up-to-date case records in accordance with agency standards, accreditation and funding source requirements

Case Management:

Coordinate treatment with relevant staff both inside and outside the agency. Communicate observations and information about clients to the program supervisor, prescriber and treatment team. Advocate for consumers and makes indicated referrals

Productivity:

Clinicians are expected to spend 65% of their time in face to face consumer contact and to provide routine productivity reports to their supervisor. All schedules, statistical reports/logs, and clinical reports are to be submitted within proscribed timelines. Documentation should be completed in electronic health record within 24 hours of service delivery.

EISS Specific responsibilities:

Participates in Mobile Outreach responses for the initial evaluation and referral of consumers to the EISS program. Provides crisis intervention

OTHER DUTIES

Participate in the Quality Improvement process and specific quality initiatives for the program. Utilize supervision and training as opportunities for enhancing professional competence. Perform all duties in a professional and ethical manner consistent with agency, licensing, COA, and professional standards. On-call duties on a rotating schedule with clinical team. Other related duties as assigned.

KNOWLEDGE/SKILLS/ABILITIES:Bilingual English/Spanish is desirable. Computer literate

Position Requirements

MINIMUM QUALIFICATIONS: Master’s or doctoral level degree in mental health field. Two years treatment experience in mental health and substance fields with adults, children and families is preferred

LICENSES/CERTIFICATIONS NEEDED:  Clinical license required. LSW/LAC/LMFT (LCSW/LPC preferred
