Richmond Behavioral Health Authority is hiring for a part time qualified and highly motivated individual(s) to join our Adult Mental Health Team. This individual will perform in clinical and administrative support of the Mental Health Docket (MHD) and Behavioral Health Docket (BHD) to ensure that the goals for the programs are met. The Mental Health Docket Case Manager assesses needs, develops, implements, and monitors service delivery and Individualized Service Plans (ISP), coordinates and monitors services with other agencies, counsels and assists clients, maintains records and files, prepares reports, and performs related work as apparent or assigned. Work is performed under the limited supervision of the assigned supervisor.
Essential Functions
Manages or assists with case load of clients as assigned by supervisor; eligible clients include but are not limited to referrals to the MHD/BHD, enrolled MHD clients, and enrolled BHD clients.
Assists the MHD Assessment Clinician and the BHD Assessment Clinician in the assessment process as needed.
Prepares a collaborative, client-centered Individualized Service Plan (ISP) based on recommendations of the Mental Health Assessment, results of the Criminogenic Risk Assessment, and clients' self-identified goals.
Collaborates with Probation Officers as well as all relevant members of the docket teams to monitor/update clients' case plan/Individualized Service Plan (ISP) and recommend appropriate sanctions/incentives to encourage participant engagement.
Facilitates cognitive behavioral and/or psycho-educational groups based on evidence-based curriculum to address client needs in areas to include but not limited to trauma, coping skills, anger management, and substance use.
Plans and facilitates group sessions with clients participating in community activities.
Coordinates case management services and provides individual clinical treatment that is person-centered and trauma-informed in office, home, jail and community settings.
Case management tasks includes but are not limited to connecting clients to a continuum of mental health (outpatient counseling, psychiatric care, medication management, and crisis services) and SUD treatment (withdrawal management, medication-assisted treatment, residential [gender-specific, co-occurring disorders] and outpatient services).
Monitors clients' level of functioning and assesses need for crisis intervention.
Maintains awareness of the sensitivity of the information provided to the court and provides only the most relevant information needed by decision-makers.
Documents and charts individual progress notes, group progress notes, discharge summaries and other client related data, correspondence, medical records, etc. in Electronic Health Record (Profiler) in accordance with RBHA policy.
Attends and participates in a variety of meetings to include clinical, administrative, and supervisory meetings.
Represents clients and Authority in court and legal proceedings.
Provides transportation assistance, as needed.
Completes all required continuing education to remain in compliance with standards set forth by the licensing board.
Remain current on RBHA mandated training. Seek opportunities for professional development. Completes electronic timesheets and expense sheets.
Collects and compiles statistical data to prepare monthly reports.
Updates client files, database records and various tracking documents as needed.
Maintains an inventory of office supplies; orders supplies as needed.
Creates and compiles expenditure spreadsheets from invoices; verifies accuracy of invoices; reviews and submits reimbursement and refund requests.
Organizes, reconciles and distributes multiple daily schedules; schedules and reschedules appointments for clients as required.
Creates marketing items for client referral as needed including greeting cards, presentation board for events, etc.
Files and scans various documents.
