The social worker will assess people living in nursing homes who may be able to live in the community with supportive services, develop person centered care plans and coordinate activities to support their move into the community. Bring your experience and passion to support the independence and well being of our clients! In the field about 2 to 3 days per week, typically in suburban Cook county.
