Under the supervision of the appropriate supervisor or manager, the Home Health Aide provides personal care services consistent with agency standards of care, policies, and regulations.

3. Provides services consistent with the established plan of care for the Home Health Aide.
a. Reviews care plan prior to visit.
b. Documents care that the RN assigned.
c. Communicates with Clinical Supervisor/Manager to facilitate changes in patient condition/status when observed by the Home Health Aide.
d. Promotes self-care when established on care plan.
e. Reports changes in client's condition to the Clinical Supervisor/Manager within the required time frame established by the agency.
4. Performs only those assigned procedures that have been approved and signed off by the RN on the skills checklist.
a. Complete Skills Checklist in a timely manner.
b. Demonstrates attempts to complete skills checklist.
5. Assists client and/or caregivers maintain a clean and safe environment in the home.
a. Observes environmental barriers and assists caregivers/clients with changes to promote safety.
b. Observes cleanliness of environment and communicates concerns with Clinical Supervisor/Manager.
c. Reports concerns to Clinical Supervisor/Manager in a timely manner.
6. Provides first aid consistent with the American Heart Association or American Red Cross standards when appropriate.
a. Knowledgeable of responsibilities in crisis situation.
b. Responds to crisis situation effectively.
c. Notifies Clinical Supervisor/Manager of incident.
7. Documents and submits records of activities.
a. Required charting is completed on a consistent basis.
b. Notes and time sheets are consistent.
c. Documentation is submitted according to agency policy.
8. Maintains knowledge of and adheres to infection control policies.
a. Reports observations of suspected infections to the Clinical Supervisor/ Manager timely.
b. Disposes of contaminated waste per policy.
c. Practices Universal Precautions and infection control measures when performing client care activities.
9. Maintains client confidentiality.
a. Discusses client care issues only within agency.
b. Handles client record in safe manner, protecting confidentiality.
10. Maintains clear communication with RN Clinical Supervisor(s).
a. Communicates on a regular basis with Clinical Supervisor/Manager.
b. Assists in coordinating HHA Supervisory visits.
11. Demonstrates evidence of clinical growth.
a. Maintains continuing education hours.
b. Attends any mandatory programs held at agency.
c. CPR certification preferred.
12. Performs other related duties as required.
a. Assists in the orientation of staff and others.
