Valley Community Services Board serves as the single point of entry into publicly funded mental health, developmental, and substance use disorder services for these localities.

Description

VCSB is looking for a Permanent Support Housing (PSH) Mental Health Case Manager! The PSH Case Manager provides ongoing mental health case management to PSH participants experiencing severe mental illness (SMI).

Summary of Essential Functions

  • Collaborates with PSH Specialist to promote housing stability.
  • Provides comprehensive initial and annual assessment of client strengths and needs.
  • Participates in development of the consumer service plan.
  • Refers, links, and coordinates Medicaid and non-Medicaid services.
  • Provides supportive counseling and case management services.
  • Provides crisis intervention and crisis stabilization services as needed.

Minimum Qualifications

All applicants must meet the following minimum qualifications in order to be considered qualified for the position:

  • QMHP certification,
  • One year’s clinical experience with adults with SMI, and
  • Valid Virginia driver’s license with an acceptable driving record.
 
Position Requirements  
Salary Information $42,751.00 to $51,301.00 annually, plus benefits.
