We find individuals suffering with or at risk for chronic health problems, we selectively couple them with an expert MOBE Guide in an empowering relationship and we engage them in a journey of better ways for thinking and acting toward enhanced physical, emotional and social wellbeing. This involves introducing them to entertaining, compelling and actionable mental and physical exercises.

Position Summary

The MOBE Clinical Pharmacist provides telephonic Comprehensive Medication Management / Optimization services to MOBE Participants, identifying, preventing and resolving medication therapy related problems (using the Indication, Effectiveness, Safety, and Adherence framework), via communication with providers and participants nationwide. The MOBE Pharmacist is also available to address ad-hoc questions from our Participants, or emerging medication-related needs as they arise, collaborate with the multi-disciplinary MOBE Guides who help our Participants improve their health through incorporating healthy behaviors in their life, and serves as a 'Pharmacist Advocate' for our members to help them maneuver the system through information or on their behalf.

You will educate Participants on their medications, how to get the best results from their medication experiences, current and future, and how to modify habits and interactions with the health care system, that will result in optimized results and improved health outcomes. You will connect and advise Providers on areas of concern and re-engage them on the participants medication-related health.

These services are designed to improve the well-being of our members, empower them with information to get better results from the health care system and take care of their own lives, as well as optimize medication utilization at the individual level. MOBE Pharmacists provide their services to individuals with complex patterns of health care utilization, and often have multiple chronic conditions and combinations of drug use that may significantly impair their quality of life or wellbeing.

Responsibilities:

  • Provide telephonic Comprehensive Medication Management services to MOBE Participants to help optimize their medication experience, aligned with MOBE's approach to help individuals better self-manage their health.
  • Collaborate and consult with MOBE Guides to provide the best impact for Participants
  • Provide personalized education and information, through multi-media and telephonic interactions with MOBE Participants, and their providers, as relevant.
    • The Pharmacist will use motivational interviewing techniques to promote self-management principles and empower members to achieve optimal medication-related outcomes in conjunction with their health care providers.
  • Stay abreast of clinical guidelines and research developments for drug, dietary supplement/vitamin, and herbals use.
  • The position may have opportunities to contribute to projects related to data analysis, program and process development, medical writing, and clinical educational content development.
  • Precepting pharmacy students when on APPE- ambulatory care or elective rotations, and/or student interns.
  • Obtain, and maintain in good standing, various state licensures, depending on business need.
  • May be required to assume Pharmacist in Charge (PiC) responsibilities, with administrative support, based on specific state needs;
    • Represents MOBE with state Boards of Pharmacies during licensing and/or audit(s) when needed, supports policy and process documentation, and review consistent with respective state(s) requirements, maintain good standing for each state assigned.
    • Provide compliance oversight of all team members engaged in remote pharmacy advisory services in relevant PiC states.
    • Collaborate with other internal stakeholders to ensure the pharmacy is fully compliant with federal and state regulations for all healthcare services programs for the applicable state.
    • Respond and act upon audit results, as necessary.
  • Other duties as assigned to help fulfill our mission and abide by MOBE's Guiding Principles.
 
Position Requirements

Qualifications:

  • Candidates must possess a pharmacy degree from an ACPE-accredited institution and have direct experience providing ambulatory care services
    • PGY1 residency or equivalent (3 years) direct patient care, providing enhanced MTM or CMM (preferred) experience required.
    • PGY2 residency in ambulatory care or psychiatry experience preferred, but not required.
    • Board Certified in Pharmacotherapy (BCPS), Ambulatory Care (BCACP), or Psychiatry (BCPP) preferred, but not required.
    • Experience in Geriatric Pharmacy or Board Certified in Geriatric Pharmacy (BCGP) desired, but not required.
  • Pharmacy licensure in MN or other state and successful attainment of MN license within 3 – 6 months of hire, in good standing, required. If not licensed in MN at time of hire, will be required to register as a Pharmacist-intern with the MN Board of Pharmacy before start-date and successfully attain MN license within 3 months of hire.
  • Willingness to obtain and maintain multi-state licensure(s), and/or PiC status for various states, as depending determined by business need, required.
  • Candidates must have a high level of ambulatory practice skills, enthusiasm for patient care, and the interpersonal skills needed to interact effectively with health professionals, patients, and other colleagues. The candidate must have a positive attitude toward the profession of clinical pharmacy and its role/potential within the broader health professions.
  • The candidate must have good written and verbal communication skills. The candidate must be adept in problem-solving and analytical skills.
  • The candidate must be a good listener and display empathy for medication-related needs and understand the importance of communicating in such a manner in member interactions.
  • The candidate must be proficient in MS Office suite and be capable of learning and implementing use of new technologies/documentation tools.
  • Hours are Mon-Fri, shifts are flexible based on business need. Willing to work 1 evening a week, desired.
  • English + Spanish multilingual desired but not required.
