As a Community Health Worker you will connect Clinton residents to the medical, behavioral health, and dental services they need most while providing education and outreach in a way that is culturally appropriate for a variety of communities within the QCA.
Starting rate for this position is $15/hr.
Community Health WorkerDuties Include:
- Providingcommunity outreach and education to coordinate healthcare and community resources for targeted populations.
- Pandemic health navigation including COVID-19 education, outreach, contract tracing, and coordination of care and community resources
- Initiate contact via preferred contact workflows and defined time frames set by IDPH & CHC
- Initiate management of Resource Requests within defined time frames set by IDPH & CHC
- Ensure services are provided in targeted communities
- Coordinate health care and community resources for clients
- Assist patients with establishing CHC as a Primary Care Provider
- Utilize interpretation for any interviews of non-english speaking clients
Requirements for a Community Health Worker:
- Excellent computer and communication skills. Attention to detail and the ability to manage multiple projects/tasks and work independently.
- Expierence working with insurances (Medicaid, Medicare & Private Insurances)
- Community involvement (past work or volunteer experience) and knows the resources in the community.
