Since 1971, Hampton-Newport News Community Services Board has provided a comprehensive continuum of high-quality, essential, and community-based services to citizens of the Peninsula who are affected by mental illness, developmental disabilities, and substance use disorders.
Case Management is an essential and unifying factor in the H-NNCSB’s service delivery system. The Case Manager serves as the single point of contact and support for the family and child regardless of the level of treatment. The Case Manager provides ongoing assessments of needs, linking and coordination of services and monitoring of services for young people with serious emotional disturbances or substance use disorders. Activities include interagency planning, collaboration and treatment monitoring to assist families in accessing social, educational, vocational and other supports.
The Case Manager ensures continuity of directly provided and contractual services to agency's mental health, developmental disabilities and substance use disorder client populations. Case Managers assist clients with their basic physical, financial, social and personal needs. Case management services for the client include outreach, psychological and need assessments, treatment planning and service provider linkage, monitoring, advocacy, counseling and crisis intervention, daily living support, medication supervision, and similar functions.

Requirements: Bachelor’s degree in Human Services (Social Work, Psychology, Sociology, or Counseling) with one (1) year experience in case management services.
Valid Driver’s License.
