The Medical Social Worker monitors the patient's progress throughout hospitalization and documents relevant findings concerning Social Services and discharge planning promptly. Establish accountability for data-driven quality care, service, and financial outcomes related to patient transitions of care. Set priorities for improving processes and outcomes related to patient care transitions. Consider outcomes focusing on appropriateness, efficacy, efficiency, timeliness, accessibility, continuity of care, the safety of the care environment, and respect for the patient's values, preferences, and beliefs.   Position Requirements MSW with 1-year experience providing social work services in an acute care setting preferred. Must have basic computer literacy. Working knowledge of Microsoft Office, clinical database software, MIDAS knowledge is a plus. Strong leadership ability and excellent interpersonal skills, as well as, solid oral and written communication skills.   This position is currently not accepting applications.
