Quality Improvement/Quality Assurance (QI/QA) Officer

Overview: The QI/QA Officer provides leadership to the accreditation and quality functions for the health department. In addition, this position will plays a key role in the creation of the infrastructure required for achieve the highest quality of service delivery and accountability. This position is responsible for leading and assisting all programs within the Lower Elwha Health Department to develop, monitor, and enhance continuous quality improvement and meet all required Federal and Washington State regulations. The QI/QA Officer assists all health programs to meet accreditation standards and develop a culture of continuous quality improvement.

WORK LOCATION: Lower Elwha Tribal Health Clinic

SUPERVISED BY: LEHD Operations Director

POSITION STATUS: Regular Full-Time

SALARY LEVEL:

WORK SCHEDULE 40 hours per week, with ability to flex schedule coordinate with other staff and clinic demands.

ESSENTIAL FUNCTIONS OF JOB:

  • Work in coordination with and under the supervision of the Operations Director.
  • Manages all aspects of Accreditation process to include: survey preparation, clinical and administrative reviews, and survey application submission. Develops and monitors corrective action plans as required.
  • Demonstrates ability to interpret accreditation standards, identify organizational gaps, and implement processes to meet standards and/or close gaps.
  • Owns and leads accreditation management to include the development of detail work plans, setting deadlines, assigning responsibilities, and monitoring/summarizing project progress.
  • Works closely with Program Managers and supervisors to assure all required documentation is obtained and kept up to date on designated electronic storage sites.
  • Develops, organizes, reviews, and updates policies & procedures.
  • Collects and organizes documentation throughout the department for ongoing program needs or develops documents, as needed, to meet needs.
  • Conducts training and coaching to facilitate understanding of the accreditation requirements within the department and with patients.
  • Develops and monitors Continuous Quality Improvement (CQ) activities including Health Plans in collaboration with the Operations Director and Health Services Director and all program managers.
  • Chairs the Quality Improvement Committee; maintains minutes of the Committee meeting.
  • Ensures LEHD Committees are scheduled and minutes from committee meetings are recorded and maintained.
  • Functions as Health and Safety Advisor - attends meetings
  • Functions as, oversees or supports Infection Prevention efforts and initiatives; ensures all efforts are appropriately documented and recorded.
  • Functions as Risk Manager.
  • Functions as Credentialing Coordinator; ensures LEHD providers are appropriately paneled and credentialed with all necessary licensure and payer entities.
  • Assists in performing clinical compliance audits; ensures peer review process is completed and feedback as appropriate is provided.
  • Attends management meetings/functions as required to support Health Department goals and activities.
  • Assume other duties as assigned by Operations Director and/or Health Services Director.

OTHER DUTIES AND REQUIREMENTS:

  • Creates and updates an annual Quality Improvement/Quality Assurance Strategic Plan
  • Provides reports for the Operations Director and/or the Health Services Director and the Business Council.
  • Provides in-service's to staff to maintain compliance with State and Federal regulations
  • Attends trainings to maintain a current knowledge of roles required for position

 
Position Requirements

DESIRED QUALIFICATIONS:

  • Minimum of an Associate’s Degree in a related field
  • Five (5) years’ experience in an ambulatory care setting
  • Has held primary role in accreditation activities in an outpatient setting
  • Basic computer skills-Word, Excel, PowerPoint
  • OSHA/CLIA knowledge/training
  • BLS/CPR certification
  • AAAHC or JCAHO knowledge/training
  • Infection Control knowledge/ training
  • Credentialing of physicians, ARNP, PA-C, and Mental Health providers- knowledge/ training
  • Basic knowledge of medical office procedures, billing and terminology
  • Ability to work as team member with medical staff
  • Ability to maintain and respect strict confidentiality
  • Ability to maintain friendly and courteous manner in all patient and staff encounters
  • Ability to read, understand and follow oral and written communication
  • Ability to communicate clearly orally and in writing
  • Ability to operate computers, fax machines, telephones and photocopy machines
  • Ability to be versatile and assume varied roles depending on needs of the moment
  • Appropriate professional appearance and behavior at all times
  • Ability to perform ALL of the responsibilities listed in section above
  • Must pass Background Check and UA Drug testing prior to employment

DESIRABLE QUALIFICATIONS:

  • Experience working within a Native American or Tribal health care system
  • Familiarity with ambulatory care electronic health record data systems
  • CARF accreditation knowledge/ training

PHYSICAL DEMANDS:

  • Requires sitting for extended periods, standing for presentations, stooping and bending
  • May require lifting up to 50 lbs.
  • Use of basic office equipment such as computers, telephones, fax machines, or photocopiers

WORKING CONDITIONS:

  • Work is performed in primary care outpatient clinic environment
  • Interpersonal contact with staff and patients is essential
