Description

The Social Prescribing Navigator (SPN), connects clients to appropriate resources based on clients’ identified interests and needs, and supports their journey to full well-being. The SPN bridges the gap between clinical and social care while working with inter-professional health providers to refer clients to community-based programs. The SPN provides crisis intervention, outreach, information, support, advocacy and short-term supportive counselling to clients of the Social Prescribing Program. The SPN works closely with the Crisis Intake counsellor to support this role and provides relief for this position when required.

Program-Related Activities

  • Engages and manages assessments and referrals for social activities to help enhance a sense of well-being for clients referred by inter-professional health providers and/or as a self-referral.
  • Provides risk assessments, personal and practical support, information, referral and advocacy for clients. Assesses clients’ health status by gaining an understanding of their social determinants of health.
  • Acts as an advocate for individual and systemic issues.
  • Provides crisis intervention services both in person and over the telephone.
  • Maintains information and liaisons with the Link2Feed program with the Ottawa Food Bank to address the food security clients.
  • Liaises and collaborates with staff and other community service providers to provide services and make referrals.
  • Participates in the community of practice table for social prescribing at the Alliance for Healthier Communities.
  • Participates in the development, implementation, monitoring and evaluation of treatment, education, counselling and health promotion programs and services for individuals, families and the community through the social prescribing program.

As a condition of employment, the successful candidate is required to provide proof of full vaccination against COVID-19.

 
Position Requirements

Education

  • Bachelor’s degree from a recognized university in a relevant discipline or, equivalent combination of post-secondary education and experience in Counselling. Masters prepared preferred.
  • Training in ASIST and Mental First Aid is an asset.

Professional Experience

  • Three to five years full-time relevant experience in a community setting.
  • Experience in providing community-based mental health and counselling services including crisis intervention, supportive counselling, group work, referral, and advocacy.
  • Experience and interest in working with equity-seeking and diverse clientele, low-income, high-needs, homelessness, individuals living with substance use, newcomers, older adults and a variety of multicultural communities.
  • Experience and knowledge of the fundamentals of Social Prescribing and program development, implementation, monitoring, and evaluation.
  • Experience in a non-profit, community-based organization.

Key Competencies

  • Thorough knowledge of and proficiency in current assessment, intervention and counselling techniques.
  • Thorough knowledge and understanding of community resources and relevant city service provider.
  • Networking with partner agencies and other social prescribing navigators or link workers in the community and region.
  • Excellent interpersonal and communication skills, oral and written.
  • Excellent time management and organization skills.
  • Demonstrated ability to work effectively in crisis situations.
  • Demonstrated ability to work in a multi-disciplinary team environment.
  • Demonstrated proficiency in the use of computers and various software applications, including Microsoft Office programs, client database software, and electronic record keeping in a client service setting.

Linguistic Profile

  • This position falls within Carlington CHC’s designation under the French Language Services Act of Ontario.
  • Fluency in French is required at Level (Advanced +) for oral expression/comprehension and Level (Intermediate +) for written expression.
  • Fluency in English is required.
  • Fluency in a language relevant to our client population is an asset.

Other

  • Available to work evenings and weekends if required.
  • Certification in ASSIST training and Mental Health training.
 
Salary Range $27.21/hour - $32.01/hour
