Description

JOB SUMMARY: This position is responsible for the coordination and delivery of substance abuse counseling for clients aged 12-21, designed to meet the wellness and recovery needs of the consumer. Depending on program objectives, these services may include resource development, assessments and counseling provided individually or in a group setting and may include the consumer’s family members.

 

ESSENTIAL FUNCTIONS

Intake:

Conduct initial and ongoing assessments involving the collection, analysis, and recording of significant information. Develop a treatment plan focusing on issues mutually identified by consumer and clinician.

 

Clinical Services:

Provide individual, family and group counseling to consumers as needed.

Leads educational groups.

Make collateral contacts as necessary.

Maintain communication with clients, including outreach for hard to engage clients.

Develop formal aftercare plans for clients completing a particular stage of treatment.

Prepare a discharge summary for consumers whose case is being closed.

 

Documentation:

Documentation should be maintained in Anasazi, the agency’s electronic health record software, and should include: the assessment, treatment plan, progress notes, collateral contacts, and termination summary. Documentation should also be maintained as per the requirements of meeting the agency’s licensing agreement.

 

Case Management:

Case management services are to be provided as needed and may include preparation of reports for external entities, community linkages, assistance in obtaining benefits and entitlements, referral and other client advocacy services.

 

Productivity:

Clinicians are expected to spend 60% of their time in face to face consumer contact and to provide routine productivity reports to their supervisor.

All schedules, statistical reports/logs, and clinical reports are to be submitted within proscribed timelines.

Documentation should be completed in Anasazi within 24 hours of service delivery.

 

Supervision:

Clinicians are required to participate in individual and peer supervision according to their program needs.

 

OTHER DUTIES 

Participate in the Quality Improvement process and specific quality initiatives for the program.

Interface with medical, vocational, residential and other program resources as necessary.

Utilize supervision and training as opportunities for enhancing professional competence.

Performs all duties in a professional and ethical manner consistent with agency, licensing, COA, and       

professional standards. Other related duties as assigned.

 

SUPERVISORY REQUIREMENTS:  no

 
Position Requirements

MINIMUM QUALIFICATIONS:  Masters in behavioral health field; MSW, MS, or MA in related field. Prior counseling experience required.

LICENSES/CERTIFICATIONS NEEDED:  Clinical license required or license-eligible (LCSW, LPC, etc).

PROGRAM MANDATED REQUIREMENTS:  Candidate should have LCADC or be working towards obtaining this credential.
