Description

ESSENTIAL FUNCTIONS: 

  • Design, implement and monitor individual program plans for clients
  • Assess client needs; investigate their eligibility for services
  • Counsel and aids individuals and families requiring agency assistance
  • Maintain client records and completes required documentation according to agency and accreditation standards.  This may include, but is not limited to data entry, progress notes, activity reports, time sheets and logs
  • Coordinate client activities
  • Assist clients with accessing other services (job training, counseling, housing, medical services, etc.)
  • Network with other providers to generate community resources for participants
  • May provide supportive counseling in individual and group settings
  • May transport consumers to appointments, etc.

 

PROGRAM RESPONSIBILITIES:

  • Provide intensive case management services to clients in need – focusing on self sufficiency, housing needs, advocacy, trauma-informed care and hands-on assistance
  • Provide transportation and accompaniment/advocacy for clients working with outside agencies, when necessary
  • Assist Program Supervisor with the spend down of direct client financial aid
  • Facilitate DV counseling group sessions as directed
  • Offer weekly intake appointments at the Outreach program for general population clients
  • Attend scheduled case conferences at both the Outreach and Safe House
  • Assist in bridging the gap between Safe House and Outreach services via group attendance and one on one discussions with clients who could benefit from ongoing DV counseling post Safe House
  • Ensure that eligible victims are educated about their right to apply for assistance and relief via the VCCO (Victims of Crime Compensation Office). Provide hands on assistance with applications, as needed
  • Administer the Parenting Stress Index with clients who have children
  • Maintain accurate and timely documentation of client records and reporting as needed

Position Requirements

MINIMUM QUALIFICATIONS:  High School / GED + 5 years of human services experience; OR relevant professional credential such as CPRP, CRC, CADC + 2 years related exp., OR a combination of one or more years of college + related work or life experience together equaling four years; OR currently being in the last semester of college working towards a Bachelor’s degree in the human services field; OR Bachelor’s degree in human services field, rehabilitation, nursing, or a related discipline.

LICENSES/CERTIFICATIONS NEEDED:  Valid driver’s license required.

MANDATED PROGRAM REQUIREMENTS:  Providence House is a 24-hour residential program, as such requires staffing at all times.  When accepting a position with this agency it is important to know that the regular shift at the time of hiring is subject to change during the course of your employment, and that at times of staff emergencies or special events you may be required to fill shifts other than (instead of) your regular shift. This may include holidays.
