BSW/MSW, Care Specialist needed for full-time temp or temp to perm position in the Bronx. Hybrid work schedule of part-time in their office, work from home, and occasional home visits.
The care specialist works for the Caregivers Program, providing support to caregivers who are caring for an ill, frail, disabled or cognitively impaired individual with Alzheimer's or other forms of Dementia. The Care Specialist provides client assessments, individual counseling, care coordination, educational training, respite services, supplemental assistance, information & referral and performs outreach initiatives to existing and prospective caregivers.
The Care Specialist role is vital in ensuring that caregivers and their families receive the information, support, and resources they need to cope with the challenges of caregiving.
