Candidates must have a BSW or MSW along with a strong interest in empowering older adults to age in the community through an array of supportive services. The Older Adult Care Manager will maintain a caseload of clients to include (not limited) to the following duties: Provide community-based care management services to older adults and/or their caregivers. Develop a client-centered goal plan appropriate to needs of the person served. Implement trauma-centered interventions with the client. Coordinate services with community providers, caregivers, and/or other family members.   Position Requirements Candidates must have a bachelors and/or masters degree in social work from an accredited school. Licensed strongly preferred but not required. Work hours are Monday-Friday, daytime hours.
