Mental Health Case Manager - Flexible Supports 

About the Organization York Support Services Network (YSSN) is a non-profit agency providing streamlined access for mental health and addictions services as well as case management and community crisis response services within York Region and South Simcoe. It is a designated administrator of the Developmental Services Ontario program and offers access for adults to developmental services in central east region.

One Full-time Permanent Position & One .5 Permanent Position (17.5 hours per week)

This position will provide intensive case management and advocacy services to individual’s age sixteen and over who are experiencing mental health, dual diagnosis and/or addictions and who are vulnerably housed or homeless. Through a collaborative approach, this position will facilitate recovery and assist service users who are experiencing significant difficulties in daily functioning to improve their quality of life. Tasks will include direct service interventions, clinical support and community development. Daily travel within York region will be required.

Position Requirements
  • Bachelors degree in Social Work or related discipline (i.e. Health Sciences)
  • Minimum 3 years related experience required
  • Proficiency in Cantonese and/or Mandarin is considered an asset
  • Knowledge and practice of psychosocial rehabilitation approach and advanced clinical and assessment skills are assets
  • Counseling, crisis prevention and intervention experience
  • Experience working with individuals having serious mental illness is essential
  • The successful applicant will be required to provide a police clearance that meets agency standards
  • A valid G driver's license, appropriate insurance coverage for business use, and reliable vehicle are required for this position
