Specific Job Detail

General Responsibilities

  • Ensures timely transition of referrals into admissions identifying barriers and solving customer service and other issues through partnership with the clinical and operations teams.
  • Regularly and customarily spends time engaged in coordination of referral-to-admission process from initial inquiry to scheduling of admission.
  • Receives fax communications, makes copies and distributes to clinical team as necessary.
  • Maintains a thorough knowledge of the agency products and services, acuity capabilities and referral relationships.
  • Gathers and records data base information during the intake process.
  • Ensures the maintenance of up-to-date account information for existing referral sources (i.e. address, contact, preferences, etc.) and maintains referral source preference information.
  • Provides information for reports that track statistics and trends for sales manager, administrator, DPS, as requested.
  • Communicates and educates referral sources regarding services via written and telephone contacts.
  • Investigates, verifies, negotiates, and documents coverage from all payer sources.
  • Understands and applies the policies of Heartland Home Health and Hospice and regulations of Federal and State regulatory agencies as they apply to the intake processes.
  • Provides information for the morning stand-up meeting to discuss daily census, referrals, admissions, referrals pending, and non-admits, discharges, and issues/concerns relating to the intake process.
  • Maintains an educated understanding of all payer sources and impact on revenue.
  • Communicates appropriate information to team members regarding new referrals and admissions including patients' needs for equipment, supplies, medications, and plan of care.
  • Gathers and tracks the receipt of appropriate orders and documentation of eligibility per corporate policy.
  • Coordinates timely follow-up on referrals not admitted and pending referrals. Uses an inquiry management system to prioritize and create an appropriate follow-up cycle.
  • In coordination with the Office Manager, ensures that admission packets are available for admission team members.
  • Assists in making follow-up calls to referral sources and patients/families.
  • Assists in mailing welcome and thank you letters to patients/families and referral sources.
  • Consistently employs Impact Selling techniques to lead to appropriate patient, family, or referral source commitment.
  • Actively manages and can demonstrate strong working referrals relationships based upon regular communication.
  • Actively seeks out ways to expand the number of referrals from referral sources who call in.
  • Be available to participate in weekly growth meeting including reporting of progress of actual vs. budgeted SOCs and census.
  • Coordinates scheduling of personnel for onsite information gathering, patient evaluations, patient consents, and admissions.
  • Provides excellent internal and external customer service and promotes appreciation of customers with other staff.
  • Maintains referral/admission, discharge, follow-up tracking boards.
  • Meets or exceeds established productivity standards.
  • Performs any miscellaneous work assignments as may be required.
  • Ensures growth and profitability of the company through the responsible use of company resources.
  • Acts as a brand champion understanding and articulating Heartland differentiators.
  • Files documents as per policy.
  • Adheres to established personnel policies.

The Admission Coordinator is an internal customer service position responsible to coordinate the referral-to-admission process for home health, hospice, and infusion patients. Provides administrative support to admission team to ensure that all eligible patients receive care, barriers to admissions are addressed and resolved and agency(ies) growth and profitability is maximized.

Educational Requirements Associate Degree in related field required. Requisite experience may be considered in lieu of Associate Degree. Bachelor degree preferred.
