Position Summary:
Under general supervision, provides support for the Social Work Services Department staff as a social work associate in the performance of various duties including screening, case management, referral services and other related duties.
Scope of Work: This position is located in the Social Work Service Department of the Tohono O’odham Nation San Xavier Health Center, San Simon Health Center and the Santa Rosa Health Center. As the Social Work Associate, the incumbent will be trained in all aspects of medical social work. The incumbent works under the supervision of the Supervisory Social Worker.
Essential Duties and Responsibilities: (Depending on the area of assignment, an incumbent may not be required to perform some of the duties listed below):
• Performs various screening and assessment functions, including taking social history information, assessing personal and family resources, providing instructions to individuals and families in obtaining assistance.
• Refer cases requiring trained professional intervention.
• Provide supportive case management and referral services to individuals and their families with problems related to financial assistance, transportation, burial arrangements, family planning, long-term care, housing, disability, and durable medical equipment.
• Schedules and makes home visits to individuals and families living within the Tohono O’odham Nation to continue assessment and monitoring activities.
• On occasion, may participate in case management monitoring visits to residents temporarily placed in skilled nursing facilities.
• Participates in hospital continuity care meetings, treatment planning conferences as needed and provides social work consultation to various Hospitals and Clinics, and Community Health staff.
• Maintain accurate and timely records of all patient contacts per The Joint Commission and departmental policies and procedures.
• Makes and accepts referrals, provides assistance and consultation in cases involving behavioral or medical health problems.
• Works closely with medical, nursing, and professional social work staff to resolve issues with post-hospital care, planning, continued treatment, and development of third-party resource assistance.
• Screens walk-in referrals to determine the nature of the problem and make appropriate referral disposition.
• Provides necessary assessment and intervention without further referral when indicated.
• Develops and maintains own caseload under the guidance and supervision of the supervisory social worker and in conjunction with the social work program’s mission.
• Assist in developing a treatment plan for the identified needs of the families.
• Advocates for families in need of services according to program guidelines, policies, and procedures.
• Provides families counseling and continuity of services according to developed treatment plans.
• Identifies community resources to assist individuals and families.
• Conducts home visits to assess the home environment, identify needs, provide resources and develop an action plan.
• Collaborates with other communities, districts, law enforcement, school personnel, and other entities of the Tohono O’odham Nation to accomplish program goals and objectives.
• Travels within assigned areas, as needed.

• Performs other job-related duties as assigned and contributes to a team effort.

• Knowledge of Tohono O’odham traditions, language, history, and culture.
• Knowledge of applicable federal, state, tribal laws, regulations, and requirements.
• Knowledge of department organization, functions, objectives, policies, and procedures.
• Knowledge of the Tohono O’odham Nation’s Adult Protection Code.
• Knowledge of principles and practices of social service work.
• Knowledge of local community resources available to individuals and families.
• Knowledge of interviewing and assessment techniques.
• Knowledge of human services values and principles.
• Knowledge and general understanding of physical and behavioral health as related to social work practice.
• Knowledge of alternate financial resource development and human service helping networks in Arizona.
• Skill in providing superior customer service to external and internal customers.

• Skill in assessing situations and providing appropriate interventions.
• Ability to understand and take direction from a supervisory social work staff.
