Share Email Opening Title Case Manager Visitation  Description Looking for a full-time Case Manager Job Summary: Provide comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support services, and referral to community and social support services through person-centered care planning.  What You Will Do: Engags consumers and their families Facilitate and coordinates psycho-educational services (including groups) for consumers and their family members on a variety of topics relating to wellness (physical and mental), health promotion, prevention, skill-building and related health and wellness/recovery topics Offer on-site as well as outreach services, including transport of consumers. Fulfill case management services, including, but not limited to coordinating or directly facilitating linkages to services, service coordination, treatment enrollment assistance, and so forth Ensure consumers have community supports in place (and engage in service planning as appropriate) that fulfill a positive corroborative role Communicate observations and information about the consumer in the consumer’s service record and to team members Engage in supportive counseling and crisis intervention Participate in assessment and evaluation of consumers  Provide input into formulation of consumer-centered care plans in collaboration with team members, containing relevant goals, measurable objectives and realistic time frames for achievement; Perform reviews of progress and evaluates effectiveness of goals/objectives Participates in quality assurance activities and cross-functional action team   What We Need to See: Bachelor’s degree in a related field required Two (2) years experience working within a health care or behavioral health setting; Experience working with adults with acute symptoms of mental illness and with chronic health care conditions Substance abuse certification certified – i.e. CADC, LCADC A valid driver's license and must be insurable by agency's automobile insurance   What We Offer: 401K, no matching needed High plan insurance at a generous cost Union benefits Life and Disability insurance Per diem opportunities Excellent supervision and training Continuing education, tuition reimbursement and eligibility for loan forgiveness 10% daycare discount About CarePlus NJ: CPNJ embraces a culture of diversity and inclusion in all of it policies and procedures.
