Annual Salary $45,136

Work Schedule: 8a-4:30p, oneweekend every other month

Since 1971, Hampton-Newport News Community Services Board has provided a comprehensive continuum of high-quality, essential, and community-based services to citizens of the Peninsula who are affected by mental illness, developmental disabilities, and substance use disorders.

Assertive Community Treatment, or ACT is a team of medical, behavioral health, and rehabilitative professionals who use a team approach to meet the needs of an individual with severe and persistent mental illness. Admission criteria include severe and persistent mental illness, predominately Schizophrenia, and high service needs in order to maintain stability.

The Service Coordinator assist clients with their basic physical, financial, social, and personal needs. Service coordination services for the client include outreach, psychological and need assessments, treatment planning and service provider linkage, monitoring, advocacy, counseling and crisis intervention, daily living support, medication supervision, and similar functions.

Requirements:Bachelors degree in Human Services (Social Work, Psychology, or Counseling) with one (1) year experience in case management or service coordination services. Master's degree preferred. Must be registered with the Board of Counseling to practice as a QMHP-Aor register with the Board of Counseling to practice as a QMHP-A trainee within the first 30 days of employment.

As a condition of employment with Hampton Newport News CSB, you are required to provide proof that you are fully vaccinated for COVID 19 or fit within an exemption under the law. The selected candidate must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.
