Healthcare Center is seeking a Social Service Social Worker.. The proper candidate is compassionate, kind, and motivated. Duties include, but are not limited to Admissions and Ongoing Assessments/Documentation. Helping with Adjustment. MDS 3.0. Care Planning. Assisting with Medicaid Applications and other Financial Matters. Advocacy. Crisis Intervention. Discharge Planning.   Position Requirements NJ CSW or LSW – Must have LTC setting experience. Must be able to multitask and organize. Must have a passion for the senior population.
