Description

Do you have a desire to help others lead more fulfilling lives? Opportunity’s Adult Family Homes house four clients with various disabilities. As a assistant manager, this position is responsible for setting a good example for co-workers, maintaining good attendance, responsible and possessing a positive attitude. Assists developmentally, physically, and cognitively disabled adult individuals by providing personal cares, self-care training, implementation of behavior management plans, and accompanying support during social/recreational opportunities, either in the adult family home or the community at large. This position is responsible for the quality assurance of clients which includes but is not limited to treating clients with respect, assuring them of their rights, and providing them a safe environment to live in. This is an ideal position for someone who wants a rewarding career and has a genuine desire to help others.

ROUTINE DUTIES

  • Responsible for providing quality care, activities, and personal care to adults with disabilities
  • Mentoring and promoting independence
  • Meal planning, preparation, and assistance
  • Maintains records of services performed and of apparent condition of resident/participant.
  • Works with Supported Living Manager to develop and coordinate behavior plans and outcome measurements for residents/participants.
  • Maintains a cooperative relationship with families and guardians of residents/participants, as well as with team-members within your department and other agency departments, and with all collaborating community agencies and service providers.
  • Assures all charting is up-to-date and signed off on in resident’s binders. (Charting includes progress notes, activity logs, goal sheets, behavior tracking, medical records, range of motion, weight charts, menus, blood pressure tracking)
  • Responsible for all end of the month paperwork (pulled and copied, progress notes, goal sheets, behavior tracking, med records, etc)
  • Maintain a cooperative relationship with families and guardians of residents/participants, as well as with team-members within your department and other agency departments, and with all collaborating community agencies and service providers.
 
Position Requirements

QUALIFICATIONS:

  • Must be at least 21 years of age, have a valid driver's license for at least 3 years, proof of personal vehicle insurance, and a good driving record
  • Ability to pass a criminal and caregiver background check
  • Be dependable, responsible, and reliable
  • Experience preferred

Opportunity Inc. was founded as United Cerebral Palsy of North Central Wisconsin in 1952 by a small group of parents and other concerned individuals to promote awareness and educate the public about cerebral palsy. A landmark decision occurred in 1978 to shift the agency’s focus to serving children and adults with any kind of disability. In 2004, the agency’s name was changed to Opportunity Inc. to better reflect the services we provide and the people we serve. Opportunity has grown from an agency with three staff members providing day services to six clients, to a professional specialized staff providing a wide array of services in multiple counties in Wisconsin.
