General Purpose:

Under general supervision, provides professional, on-going analysis of the department’s programs, structure, policies, procedures and systems. Represents the department, serving as lead on projects and serving on regional, and multi-agency committees. Presents results and recommends ways to integrate new processes, operational changes and required training.

 

Essential Functions:

(These duties are a representative sample; position assignments and detailed work instructions may vary.)

 

  1. Conducts public health planning projects related to County strategic plans and community health assessments; collects data, administers and tabulates surveys, manages focus groups, and conducts key informant interviews.
    1. Develops, maintains, and facilitates a sustainable health improvement implementation action plan, including specific strategies, milestone activities, timeline, commitments from partners, evaluation and reporting.
  2. Serves as project lead for Community Health Assessment and Health Improvement Plan process, including develops and implements timelines, master planning schedules, and community involvement process that assures the active involvement of community members and stakeholders in the community health assessment and planning process.
  3. Serves on regional, and multi-agency committees involved in assessment and planning.
  4. Conducts program evaluations; compiles and analyzes information and develops reports on department plans and community needs.
  5. Participates in Department strategic planning, and serves as a lead in facilitating, and documenting strategic planning process.
    1. Serves as department quality improvement and performance measurement coordinator. Collects data, develops measurement tools and conducts analysis to support quality improvement processes.
  6. Develops, implements and evaluates evidence-based public health programs, practices, policies and systems; evaluates program status and when possible, works to align with Public Health accreditation board standards for all population-based services.
  7. Uses a Health Equity lens in all work and understands social determinants of health.
  8. Identifies potential sources of funding for additional programs, education and other resources identified in the strategic plan.
  9. Recommends and develops policies and procedures.
  10. Prepares and delivers reports and presentations.
  11. Participates on various committees and other user groups involved in public health planning.
  12. Performs emergency response roles, as requested.
  13. Performs other duties of a similar or related level and assists public health leadership as necessary or assigned.
 
Position Requirements

Minimum Education: B.S. or B.A. in Public Health, Health Promotion, Health Sciences, Data Analytics, Statistics, Epidemiology, or related field. Master’s degree in Public Health, Health Science, Health Administration, or related field preferred.

Minimum Experience: 3 years post-graduate related experience OR A Master’s degree in Public Health, Health Science, Health Administration, or related field may be substituted for 2 out of the 3 years of experience.

Licenses/Certifications: Valid Driver’s License.

 

Job Qualifications:

Knowledge of:

  1. Basic principles, practices and objectives of public health, communicable and chronic diseases.
  2. Purpose and role of County government in public health rules and regulations.
  3. Core public health functions and essential services.
  4. Conducting research and community needs assessments.
  5. Modern office processes and procedures.
  6. Advanced principles and detailed technical aspects of public health program areas.
  7. Public health needs and problems in the community.
  8. Policies and procedures for public health work in the community
  9. Relevant computer hardware and software applications (Microsoft products used).

 

Ability to:

  1. Speak, read and write the English language.
  2. Communicate effectively verbally and in writing.
  3. Relevant Federal, state and local laws, rules, regulations, codes and/or statutes.
  4. Advanced principles and detailed technical aspects of public health program areas.
  5. Methods and techniques of research, statistical analysis and report presentation.
  6. Drive vehicle to perform one or more essential functions.
  7. Financial management, including basic bookkeeping and accounting practices, procedures and systems.
  8. Report writing and communicating findings to the public.
  9. Grants, grant writing, contracts, and legal agreements.
  10. Read, analyze and interpret professional journals or periodicals, procedures and government regulations.
  11. Define problems, collect data, establish facts and draw valid conclusions.
  12. Use resources to organize and plan and to make decisions and solve problems.
  13. Establish effective working relationships with employees, team leads, supervisors, other Departments/Offices, other agencies and organizations.
  14. Prepare and give oral presentations.
  15. Conduct research; write and/or generate reports
  16. Understand and practice safety procedures and precautions.
  17. Maintain a high level of professionalism and confidentiality.
  18. Work occasional evening and weekend hours.

Frequently interacts with other Department/Office employees. Works closely with Public Health Director and supervisor.

Frequently interacts with health-related community partners.

Occasionally interacts with general public, vendors, or other organizations.

Required Physical Activities:

While performing the essential functions of this job, the employee is frequently required to speak and understand English language, hear, and see. The employee is regularly required to sit, stand and walk. Requires eye/hand coordination and manual dexterity to use office equipment. The employee is occasionally required to lift to 30 pounds.

In times of public health emergency, the employee will be required to report for specialized duties inside or outside Garfield County once their critical personal and family emergency responsibilities have been met.

 

Work Environment:

The noise level in the work environment is usually moderately loud. There is a potential for employee to be exposed to blood borne pathogens via body fluids.

 

Required Travel:

The employee is often required to travel between offices and to other locations within the County.

 

Computers and Other Equipment:

Computer and relevant software applications, telephone system, facsimile, shredder, TV/VCR/DVD,

LCD projector, copier, and other equipment related to the work.
