The Registered Nurse assists in the delivery of primary care services and promotes healthy lifestyle choices for our clients. The Nurse provides primary care functions and participates in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the community, within the scope of practice mandated by the College of Nurses of Ontario. The nurse functions interdependently with the other members of the Primary Care team within a health promotion framework and works in a collaborative fashion to meet program goals and objectives.

Program-Related Activities

  • Performs clinical tasks by conducting nursing assessments, triage, health education/counselling, history taking, providing follow-up care, and other clinical activities, as part of a multidisciplinary team, while adhering to the College of Nurses of Ontario standards of nursing practice.
  • Ensures a fully equipped functional clinic is maintained by ordering medical supplies and maintaining inventory, ensuring equipment is maintained and sterilizing and autoclaving as needed. Ensure resources and materials are maintained in collaboration with the clinical aide.
  • Facilitates the administration of the primary care program by identifying areas where the development of protocols and procedures need improvement, participating in committee work and staff meetings, ensuring lab forms are correctly completed, maintaining the laboratory and procedures room, and monitoring controlled substances according to policies and procedures.
  • Works within a health care framework which emphasizes health promotion and education, disease prevention and recognises the special risk factors (social, physical, psychological, etc.) which may affect the health of clients.
  • Acts in an ethical and professional manner in accordance with the code of conduct of the College of Nurses of Ontario and in accordance with the policies and standards (written and unwritten) of CCHC.
 
Position Requirements

Education

  • Undergraduate degree in nursing from a recognized university or equivalent.
  • Registration with the College of Nurses of Ontario. 

Professional Experience

  • Three to five years full-time nursing experience in a community-based setting or combination of community and hospital or public health settings; experience in a primary care setting, an asset.
  • Experience and interest in working with marginalized and diverse clientele, such as low-income, high-needs, newcomer, and a variety of multicultural communities.
  • Experience in a non-profit, community-based organization an asset.

Key Competencies

  • Thorough knowledge of and proficiency in current nursing physical assessment and treatment methods.
  • Ability to apply health promotion concepts, strategies, and philosophy.
  • Ability to develop and implement programs using an inter-disciplinary collaborative approach.
  • Excellent interpersonal and communication skills, oral and written.
  • Demonstrated ability to model “client centred” service delivery.
  • High level of organizational and time management skills in a fast-paced environment. Demonstrated ability to prioritize, to be flexible, to meet deadlines.

Linguistic Profile

  • A ratio of these positions fall within Carlington CHC’s designation under the French Language Services Act of Ontario.
  • Fluency in English is required.
  • Fluency in another language relevant to our client population is an asset.
