Description

One Full-time Contract Position to March 31, 2023 (with the possibility of extension or becoming permanent)

In this role, you will be working in collaboration with Children’s Treatment Network to support children and families participating in the Ontario Autism Program who have urgent service planning needs

OAP Urgent Response Coordinators will support families of children and youth with autism who require urgent assistance with their service planning needs. This will include screening of children/youth with Autism who request the service, providing service navigation for all children and youth referred, and leading development of an Urgent Response Service Plan for those who meet the criteria and their families. They will monitor and update the plan during a child/youth’s involvement with the service. They will further support a family’s transition to other supports after their involvement with Urgent Response.

This service will primarily be provided in York Region, with flexibility for support in Simcoe County locations, as well.

This position will facilitate an inter-agency team approach to the development and monitoring of an urgent response plan that is child and family centered. The OAP Urgent Response Coordinator will also facilitate internal and external team development by understanding the Ontario Autism Program and the Children’s Treatment Network system, in general. It will work particularly closely with Service Planning Coordinators and other children’s service providers.

A hybrid remote work arrangement is available for this position

 
Position Requirements
  • Bachelors degree in Social Work or a related discipline (i.e. Psychology etc.)
  • Minimum five years’ related experience/Children’s Service System
  • Experience working with multiple special needs children, youth and their families in a case management/service coordination capacity
  • Strong working knowledge of OAP and children’s support services
  • Experience with developing an integrated service plan
  • Experience working with team members from multiple children’s sectors
  • Good working knowledge of relevant provincial policies and legislation concerning children and youth
  • Experience with team facilitation and leadership
  • Francophone or second language is considered an asset
  • Understanding of cultural diversity and second language skills are considered assets
  • The successful applicant will be required to provide a police clearance that meets agency standards
  • A valid driver’s license, adequate insurance coverage for business purposes and regular use of a reliable vehicle.
