Share Email Opening Title PT OR FT Case Manager - Healthy Families Viroqua  Description Join our team as a Case Manager with the Healthy Families Program in Viroqua!   As a Case Manager with the Healthy Families program in Viroqua you will work with a supportive team and establish a trusting relationship with overburdened families by delivering service that is culturally competent, strength-based, family centered and family directed.   PT or FT position is available!   What are the primary responsibilities of this Case Manager position? Meet with families in the least restrictive environment, typically their homes or in the community in La Crosse County Assist parents in improving their skills so as to optimize a positive home environment Increase the family's ability to problem-solve and advocate for themselves and their children Identify and refer families to additional community resources, including healthcare and other basic necessities Work cooperatively with families to establish goals and develop a plan for accomplishing those goals Conduct regular assessments of the growth and development of the children participating in the Healthy Families program Suggest, lead, and model parent/child activities Maintain written documentation of all activities and contact with families, as well as referrals or contacts made on behalf of families Participates in all agency staff meetings, team meetings, and other meetings as assigned   Benefits at FCC Hours of work are generally Monday - Friday 8:00 am - 4:30 pm Paid time off through accrued vacation and sick time & 9 recognized holidays plus an additional floating holiday!
