Orients and advocates on behalf of the residents and their families to the nursing center while offering continued support. Completes social history and MDS assessments, and develops care plan, which identifies medically related social and emotional problems and needs with realistic goals and specific actions to be taken. Facilitates discharge planning, from the time of admission, for short-term rehab patients.   Position Requirements BSW or MSW - Must have LTC setting experience. Must be able to multitask and organize. Must have a passion for the senior population.
