Social Workers will provide direct case management services to an adult mental health population in the community on a 24/7 hours basis. The position will spend 65% of time face-to-face with consumers, in their homes and the community. Social Workers Will: Facilitate and coordinate psycho-educational services for consumers and their family members on a variety of topics relating to wellness , health promotion, prevention, skill-building and recovery topics Offer on-site and outreach services (including transport of consumers) Fulfill case management services, including coordinating or directly facilitating linkages to services, service coordination, and treatment enrollment assistance Ensure consumers have community supports in place that fulfill a positive corroborative role Communicate observations and information about the consumer in the consumer's service record and to team members Participate in supportive counseling and crisis intervention Participate in assessment and evaluation of consumers at intake/admissions, during ongoing recovery/service delivery and at discharge Collaborate with team and collaborative treatment providers to establish comprehensive bio-psycho-social assessments and care plans Contribute to formulation of consumer-centered care plans in collaboration with team members, containing relevant, measurable goals and realistic time frames Perform reviews of progress and evaluate effectiveness of goals Work with administrative staff to ensure that all services are documented based on billing and regulatory standards   Position Requirements Bachelor's Degree in Social Worker OR Bachelor's Degree in Mental Health or related field, plus one (1) year post BA experience in Mental Health or related field Valid driver's license insurable by agency's automobile insurance   This position is currently not accepting applications.
