Looking for an Outpatient Care Coordinator.

Job Summary:

You will provide comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support services, and referral to community and social support services through person-centered care planning. Additionally, you will provideall services consistent with SAMHSAs fundamental components of recovery, in a person and family-centered approach that incorporates cultural appropriateness.

 

What You Will Do:

  • Ensure all project outcomes are met and reports are submitted in a timely manner.
  • Complete data collection requirements for projects and reportback to leadership
  • Work with administrative staff to ensure that all services are documented based on billing and regulatory standards; collect information on consumers and reflects all demographic data
  • Participatein quality assurance activities and cross-functional action teams
  • Serveas a liaison role for assigned partner organizations
  • Fulfillcase management services, including, but not limited to coordinating or directly facilitating linkages to services, service coordination, treatment enrollment assistance, and so forth
  • Document ervices and client information in clinical record on a timely basis with all entries authenticated,time, and dated.
  • Ensure consumers have community supports in place (and engage in service planning as appropriate) that fulfill a positive corroborative role
  • Engagein supportive counseling and crisis intervention
  • Participatein assessment and evaluation of consumers at intake/admissions, during ongoing recovery, service delivery, and at discharge
  • Provide input into formulation of consumer-centered care plans in collaboration with team members and other treatment providers, containing relevant, measurable objectives and realistic time frames for achievement


What We Need to See:

  • A Bachelors Degree in Social Work, Psychology, Counseling, Public Health, Healthcare Administration or related field.
  • 1+ years experience working in public health, healthcare setting, or mental health agency.
  • Working knowledge of EHR functions (implementation, data collection/analysis, report writing)
  • A valid driver's license is required, and must be insurable by agency's automobile insurance
