Description

Valley Community Services Board is seeking a dynamic individual to join our Adult Mental Health Case Management team. Help support our Adult MH Case Management team. Make a difference in the lives of adults experiencing mental health or co-occurring disorders. Be a symbol of leadership and a mentor to the members of the team. This position requires experience working with adults who may be dually diagnosed with mental health and substance abuse issues. This position will support the team by assisting with training, monitoring documentation, and providing supervision and supporting team members with the day-to-day responsibilities of case management.

Essential Functions

  • Provide individual and/or group supervision to assigned staff.
  • Track and provide feedback to staff regarding performance standards and productivity measures.
  • Review and monitor staff documentation withing the electronic health record.
  • Conduct chart audits each month to ensure overall compliance.
  • Monitor and review CM referrals, assign cases to staff and help manage waitlist.
  • Comply with established legislative mandates, program standards and all VCSB requirements regarding consumers rights, services and documentation of service delivery.
  • Monitor and track CNA, ISP services and treatment plan reviews within all client charts, in Peer Supportive Services and Adult MHCM.
  • Monitor caseload size, type and quality of interventions to consumers for clinical appropriateness.
  • Provide direct services and backup coverage of direct services to consumers.
  • Share the responsibility of monitoring and problem-solving any personnel, administrative, and fiscal issues.

 

 

Knowledge, Skills, and Abilities

 

Knowledge of:

  • Psychotropic medications, human personality, growth, and development.
  • Human rights policies and confidentiality.
  • MH, SA, DS and associated symptoms.
  • Basic counseling/therapy techniques.
  • Diagnosis and assessment of the seriously mentally ill.
  • Case management services.
  • Casework documentation including individualized service plans.
  • Interagency services coordination and the HUD subsidized program.

 

Skills Pertaining to:

  • Planning, evaluation, and implementation of consumer’s treatment needs.
  • Developing treatment plans.
  • Application of behavioral management principles.
  • Crisis intervention skills.
  • Providing case management services for persons with SMI and/or dual diagnosis.
  • Maintaining casework documentation
  • Facilitating/mediating and encouraging the participation of consumer’s natural support systems.
  • Clinical supervision.
  • Staff training.

 

Ability to:

  • Work effectively with all types/levels of consumers.
  • Effectively implement interagency coordination of services for the seriously mentally ill.
  • Work cooperatively with both internal and external staff and family members, and outside agencies.
  • Work a remote and flexible schedule as necessary to meet consumers and program needs.
 
Position Requirements

Requirements

  • Bachelor's or master’s degree in human services or related field.
  • 2 years clinical experience required.
  • QMHP-A or eligible preferred
  • Valid VA drivers license with acceptable driving record required.
