<p><strong><u>Qualifications:</u></strong></p><p style="margin-left:.5in">1.Must have successfully completed the C.N.A. Certification course</p><p style="margin-left:.5in">2.Must be registered on the State of Wisconsin Nursing Assistant Registry</p><p style="margin-left:.5in">3.Must be pleasant, courteous and tactful when dealing with residents, families, visitors and co-workers</p><p style="margin-left:.5in">4.Must be able to perform assigned duties independently and as a team player</p><p style="margin-left:.5in">5.Must demonstrate a positive attitude, a sense of responsibility and common sense.</p><p style="margin-left:.5in">6.Must be able to react, under supervision, in the event of an emergency.</p><p style="margin-left:.5in">7.Must be physically able to waist lift up to a maximum of 50# with frequent waist lifting of objects weighing up to 50#. Must be able to push and/or pull objects with a maximum effort of 25#.</p><p style="margin-left:.5in">8.Must be able to stand or walk for 6-8 hours in an 8 hour day.</p><p style="margin-left:.5in">9.Must be able to frequently bend, squat and twist body during an 8 hour day.</p><p style="margin-left:.5in">10.Must be organized, willing to assist co-workers and residents</p><p style="margin-left:.5in">11.Must be willing to work weekends and holidays.</p><p style="margin-left:.5in"> </p><p><strong><u>Working Conditions:</u></strong></p><p style="margin-left:.5in">1.May be subject to bruises and scratches from residents striking out.</p><p style="margin-left:.5in">2.May be exposed to infections and odors.</p><p style="margin-left:.5in">3.Involved in tasks that involve exposure to blood, body fluids, or mucous membranes on a daily routine basis.</p><p> </p><p><strong><u>Responsibilities:</u></strong></p><p style="margin-left:.5in">1.Receives assignments of duties from Charge Nurse at start of tour of duty.</p><p style="margin-left:.5in">2.Bathes residents (Bed Bath, Tub Bath, Shower)</p><p style="margin-left:.5in">3.Provides oral hygiene and denture care.</p><p style="margin-left:.5in">4.Makes beds (occupied and unoccupied), straightens rooms.</p><p style="margin-left:.5in">5.Assists at meal time, including:</p><p style="margin-left:1.0in">a.Transporting residents to and from the dining room</p><p style="margin-left:1.0in">b.Feeding Residents</p><p style="margin-left:1.0in">c.Calculating intake of meals and fluids</p><p style="margin-left:1.0in">d.Taking trays to residents eating in their rooms</p><p style="margin-left:.5in">6.Serves daily nourishments and water.</p><p style="margin-left:.5in">7.Assists in toileting or residents including:</p><p style="margin-left:1.0in">a.Assisting residents to and from the bathroom/commode</p><p style="margin-left:1.0in">b.Offering of bedpans and urinals</p><p style="margin-left:1.0in">c.Changing of clothing and linens of incontinent residents</p><p style="margin-left:1.0in">d.Peri-care to keep residents clean and fresh</p><p style="margin-left:1.0in">e.Collecting urine and stool specimens</p><p style="margin-left:1.0in">f.Emptying and recording drainage from colostomies and catheter bags</p><p style="margin-left:.5in">8.May be asked to transport resident to other departments or activities within the facility. Assist with and/or performs procedures with proper instruction such as:</p><p style="margin-left:1.0in">a.Admission of resident</p><p style="margin-left:1.0in">b.Transfer or discharge of residents</p><p style="margin-left:1.0in">c.Taking and recording vital signs (temperature, blood pressure, pulse, respirations, oxygen saturation levels)</p><p style="margin-left:1.0in">d.Assisting and care of dying or deceased residents</p><p style="margin-left:1.0in">e.Applying restraint safety devices, TED stockings, splints, etc.</p><p style="margin-left:1.0in">f.Obtaining resident weights</p><p style="margin-left:.5in">9.Provides nail care of non-diabetic residents.</p><p style="margin-left:.5in">10.Shave residents</p><p style="margin-left:.5in">11.Follows procedures for residents in isolation</p><p style="margin-left:.5in">12.Applies aseptic technique in all resident cares, including proper hand washing, cleaning of equipment to prevent spread of disease and infection.</p><p style="margin-left:.5in">13.Cleans and maintains orderliness of residents rooms, linen rooms, utility rooms, trash disposal and charting areas</p><p style="margin-left:.5in">14.Observes, reports and records in medical chart condition, alertness, appetite, etc. of resident. Reports changes or unusual finding to the nurse</p><p style="margin-left:.5in">15.Assists with restorative and rehabilitative care of residents such as R.O.M., ambulation, turning and positioning of residents.</p><p style="margin-left:.5in">16.Participates in resident care planning and utilized care plan in delivering appropriate care.</p><p style="margin-left:.5in">17.May answer telephone or intercom</p><p style="margin-left:.5in">18.Practices economy in caring for nursing equipment and in usage of nursing supplies.</p><p style="margin-left:.5in">19.Any and all other duties assigned by charge nurse, director of nursing, assistant director of nursing, or administrator for the purpose of quality resident care.</p><p style="margin-left:.5in">20.Must follow all of Holton Manor Policies and Procedures</p><p style="margin-left:.5in">21.Answer call lights throughout the facility; in each section of the facility</p>
