Join our team as a Case Manager with the Individual Placement and Support program.

 

As a Case Manager with the Individual and Placement Support program, you will be supporting clients using an evidence-based approach to support employment for people with serious mental illness. IPS helps people living with behavioral health conditions work at regular jobs of their choosing.

 

What are the primary responsibilities of this Case Manager position?

  • Assessment and treatment planning
  • Assisting clients to attain resources
  • Meet with local employers regarding employment
  • Provide skills training and transportation

 

** This position has a focus on clients with mental health needs **

Experience

  • Experience working with adults with mental health needs is preferred.

 

Education

  • Bachelor degree in human services, social work, psychology, sociology or a related field

 

Additional Requirements

  • Must be insurable & have access to personal vehicle
  • Willing to travel across multiple counties

 

License & Certification

  • Valid drivers license
