Hiring Pay Range:

CM A (Non-Exempt): $23.45-$29.31; CM B (Exempt): $54,392.00 - $67,995.20

Full Pay Range:

CM A (Non-Exempt): $23.45-$35.18; CM B (Exempt): $54,392.00 - $81,598.40

 

General Purpose:

Provide a variety of case management services to clients eligible for publicly funded long-term care (LTC) programs, including, but not limited to, Home and Community Based Service Program for the elderly, blind and disabled (HCBS-EBD), Home and Community Based Service Program for persons with brain injury (HCBS-BI), Home and Community Based Service Program for persons with mental illness (HCBS-CMHS), Home and Community Based Service Program for Children (CHCBS), Home Care Allowance (HCA), and Medicaid Nursing Facility Care.

 

Essential Functions:

Case Manager A:

Under supervision and within established guidelines:

  1. Receives referrals for LTC services from a variety of sources and performs appropriate intake and screening procedures.
  2. Provides initial client assessment and functional eligibility determination relative to the nature of the disabling condition, client capabilities, eligibility for LTC services, support systems and appropriate community resources.
  3. Develops appropriate service plan for client.
  4. Provides information and referral services to applicants who do not qualify for Medicaid LTC services in addition to providing this same service to clients who do qualify for Medicaid LTC services.
  5. Provides ongoing case management services which involve regular home visits to clients to assure the effectiveness of services, to reassess continuing need for LTC services and to determine client capacity.
  6. Provides care planning services to include development of a service plan, arrangement of alternative services, identification of appropriate support and community resources and determination of the cost effectiveness of the use of alternative services. Develops, locates and coordinates services in the community to maintain clients highest level of independence within the cost containment.
  7. Compiles information writes observations and prepares documentation as required by the agency, state and Federal requirements; maintains log notes of all contacts, significant events or activities on behalf of the client.
  8. Coordinates functional eligibility determinations with financial eligibility in the County of client residence/application.
  9. Meets all state-defined timeframes for completion of assessments, continued stay reviews and documentation in the BUS. Provides quality documentation regarding services provided.
  10. Maintains HIPAA and agency confidentiality standards and ongoing compliance with these standards of practice.
  11. Attends training relative to the client population and/or rules and regulations for LTC programs.
  12. Assists with managing the senior grant program.
  13. Reviews all work products to ensure highest level of quality.
  14. Drives vehicle to perform one or more essential functions.
  15. Performs other duties of a similar or related level as necessary or assigned.

 

Case Manager B:

  1. Under minimal supervision, performs all essential functions of Case Manager A.
  2. Assumes responsibility for more complex cases.
  3. May assist in training Case Manager A.
  4. Performs other duties of a similar or related level as necessary or assigned.
 
Position Requirements

Minimum Requirements:

Minimum Education:

CM A and CM B: Bachelor’s degree in one of the human behavior science fields; or licensed nurse if a waiver is received from the State Department of Health Care Policy and Financing;

Minimum Experience:

CM A: 1 year of experience working with long term care client population and/or Medicaid preferred.

CM B: 3 years of experience working with long term care client population and Medicaid.

Licenses/Certifications: Valid driver's license.

 

Job Qualifications:

Knowledge of:

  1. Relevant Federal, state and local laws, rules, regulations, codes and/or statutes.
  2. Human behavior related to aging, disabilities, and the LTC client population.
  3. Single Entry Point (SEP) programs.
  4. Interviewing and client assessment skills.
  5. Modern office processes and procedures.
  6. Keyboarding, data entry and word processing.
  7. Relevant computer hardware and software applications.
  8. Basic math skills.

 

Ability to:

  1. Speak, read and write the English language.
  2. Communicate effectively verbally and in writing; demonstrate good telephone skills.
  3. Abide by HIPAA rules and regulations.
  4. Effectively and efficiently provide customer service to the public.
  5. Be sensitive to individual values and needs.
  6. Work with the public including disabled, hostile and mentally unstable individuals.
  7. Handle stressful situations in a professional manner, including angry or difficult individuals; accurately assess situations and people.
  8. Interpret a variety of instructions with abstract and/or concrete variables.
  9. Make decisions incorporating information and input from various arenas.
  10. Use resources to solve problems.
  11. Prioritize and organize.
  12. Establish effective working relationships with coworkers and supervisors, other Departments/Offices, and other agencies.
  13. Understand and practice safety procedures and precautions.
  14. Maintain a high level of professionalism and confidentiality.

 

Case Manager B:

In addition to the abilities required for Case Manager A, ability to:

  1. Apply relevant Federal, state and local laws, rules, regulations, codes and/or statutes.
  2. Successfully manage more complex cases to completion under minimal supervision.
  3. Prioritize work and give adequate attention to all cases.
  4. Self-motivate and demonstrate initiative.
  5. Use independent judgment as to proper course of action and think and act quickly in emergencies.
  6. Provide leadership

Frequently interacts with the general public, community organizations and resources, treatment professionals and other organizations.

Required Physical Activities:

Specific vision abilities required by this job include close vision, color vision, depth perception, peripheral vision, and ability to adjust focus.

 

Work Environment:

The employee occasionally may be exposed to hazardous situations, which may involve physically violent persons, interviewing mentally or emotionally disturbed persons or bodily injury from biohazards, infections or blood-borne pathogens.

Employee will frequently be conducting meetings in the client’s home.

The noise level in the work environment is usually moderately loud, with frequent interruptions and multiple demands.

 

Required Travel:

Required to travel frequently within multiple Northwest Colorado counties.

 

Computers and Office Equipment:

Computer and relevant software applications; telephone system; facsimile; scanner; shredder; and copier.
