Social workers will be responsible for: Coordinating discharge plans and out of facility care if needed including transportation Preparing social histories and participating in resident care conferences Completing care plans and MDS documentation   Position Requirements Candidates must have a bachelors and/or masters degree in social work from an accredited school of social work and be licensed in Missouri (LBSW or LMSW).   This position is currently not accepting applications.
