Position: Mental Health Advocate for PACT Team (Mercer County)

Location: Trenton, NJ

Responsibilities:

As a Mental Health Advocate for our PACT team, you will:

1. Empower and Support:

  • Advocate for the rights and well-being of individuals facing mental health challenges, ensuring they have a strong voice in their treatment plans.
  • Collaborate with team members to develop personalized care strategies that prioritize individuals' goals and aspirations.

2. Community Engagement:

  • Build and maintain positive relationships with community resources, fostering partnerships that enhance our clients' access to a broad range of support services.
  • Organize and participate in community outreach events to reduce stigma and raise awareness about mental health.

3. Crisis Intervention:

  • Provide timely and effective crisis intervention, offering support to clients during challenging moments.
  • Collaborate closely with the PACT team to implement crisis prevention and intervention strategies.

4. Advocacy and Education:

  • Stay informed about mental health policies, best practices, and emerging trends to advocate for systemic changes that positively impact our clients.
  • Conduct educational sessions for clients, families, and community members to promote mental health awareness and destigmatize mental health challenges.

Full job description

JOB SUMMARY: Provides an array of mental health support services to the PACT consumer. Participates in the planning and provision of interdisciplinary training and development for all team members.

ESSENTIAL FUNCTIONS:

  • Provides support services which include but are not limited to personal care assistance (hygiene, grooming), life skill development (nutrition, money management, medication monitoring), entitlement management and advocacy, and supportive counseling (emotional support, job coach/vocational support, substance abuse support). Coordinates services with other agencies.
  • Participates in treatment planning, ongoing assessment and discharge review process.
  • Responsible for maintaining accurate and current consumer records.
  • Advocates with community service providers for on-going, emergency and other forms of health care for consumers.
  • Represents PACT, the Agency and/or consumer at various community based medical, treatment planning and service coordination meetings.
  • Contributes to and participates in the delivery of interdisciplinary in-service training and case conferences.
  • Provides on-call and emergency response (as needed)
  • Provides psychiatric/medical case management and/or specialized services to consumers as warranted and/or requested.
  • Participates in on-going supervision and/or program development with the Program Coordinator, which may consist of field visits to consumer residences, work sites, families, etc.

 

 

 
Position Requirements

Qualifications:

  • Associate's degree in psychiatric rehabilitation; OR Bachelor's degree in Behavioral Health Science, Mental Health, Psychology, Public health, or related field. + 2 years’ experience providing mental health services.
  •  Valid driver's license in good standing.
  • Ability to be on-call maybe required.
  • Demonstrated passion for mental health advocacy and a commitment to empowering individuals on their recovery journey.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively within a multidisciplinary team and engage effectively with diverse community stakeholders.
  • Understanding of trauma-informed care principles.
