Description

JOB SUMMARY:

  • Responsible for overall coordination of clinical oncology services throughout the continuum in collaboration with the multidisciplinary team.
  • Functions as a key member of the multi-disciplinary team and contributes to development of work processes, continuous quality improvement, patient satisfaction, and physician/staff satisfaction.
  • Acts as an advocate, interpreter, educator, counselor and caregiver for oncology patients and families.
  • Supports Program Growth and visibility by developing and implementing Outreach initiatives.
  • Designs and implements house-wide RN Education to support the Cancer Program.

QUALIFICATIONS:

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Care Coordination:

  • Serves as the main point of contact for patients and families.
  • Ensures appropriate patient appointments (e.g., physician office visits, tests, procedures) are made in timely manner.
  • Builds relationship with patient and families; maintains contact and keeps other care team members apprised of patient status.
  • Serves as main point of contact for referring physicians/PCPs; ensures referring physician/PCPs receive timely communication about patient status.
  • Prepares letters for referring physicians/PCPs summarizing treatment recommendations from treatment planning conference.
  • Identifies obstacles and finds solutions for issues affecting compliance with treatment plan and a positive patient experience;
  • Proposes process improvement measures as appropriate.
  • Evaluates patient psychosocial needs; Connects patient with appropriate internal or external resources.
  • Coordinates multidisciplinary conferences.

Patient Education/Empowerment:

  • Identify and document individual patient’s barriers to learning.
  • Assist patient/family to develop questions, prior to office visits, for discussion with providers.
  • Review findings and recommendations with patient/family after appointments to ensure understanding; Answer questions and communicates as needed with providers.
  • Educates patient/family about disease process, treatment options, potential side effects, and pre/post-operative education.
  • Educates patient/family about survivorship.
  • Remains available to patients/families for questions following completion of treatment plan.
  • Assists patient/family in end-of-life care decisions.
  • Develops and provides education about diet, exercise, smoking cessation and other wellness/cancer prevention and screening strategies.
  • Facilitates implementation of volunteer/patient buddy program.

Business Development:

  • Manages timely communication about patient results and treatment, with referring physicians/PCP’s, according to their personal preferences.
  • Identifies key metrics for performance including navigator productivity.
  • Prepares and distributes monthly metric reports.
  • Actively solicits feedback from referring physicians/PCPs and develops action plans to achieve physician satisfaction.
  • Collaborates with multidisciplinary team and Business Development Department to promote services.
  • Visits referring physicians/PCPs, and potential referring physicians/PCPs, regularly to increase awareness of services; educates physicians/PCPs and staff about cancer prevention and treatment.
  • Collaborates with multidisciplinary team and others to design and implement community outreach programs to increase public awareness of cancer prevalence, prevention including importance of regular screenings.
  • Develop collaborative relationships with local individuals, agencies and organizations that provide cancer education and support for cancer patients/families. •Implement and facilitate survivor support group.
  • Coordinates processes for, and provides general oversight related to ACCC (Association Community Cancer Centers) and NAPBC (National Accreditation Program for Breast Centers) accreditations to ensure the program meets requirements.

EXPECTED BEHAVIORS:

  • Knowledge of pathophysiology of specific disease processes.
  • Understanding of oncology treatment modalities and rationales including surgery, chemotherapy, radiation therapy, symptom management, survivor care and surveillance.
  • Understanding of current clinical guidelines for cancer treatment (NCCN guidelines).
  • Understanding of ACCC and NAPBC accreditation requirements.
  • Highly effective verbal and written communication skills.
  • Desire and ability to establish and maintain rapport with patient, families, physicians and other caregivers, community agencies and others.
  • Ability to plan, develop, implement and evaluate services in collaboration with Cancer Committee.
  • Ability to work autonomously.
 
Position Requirements

CERTIFICATION/LICENSE: Registered Nurse (RN)

ADDITIONAL LICENSURE/CREDENTIAL REQUIREMENTS: Graduate of accredited Registered Nursing Program.

ADDITIONAL EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Minimum 3 yrs. experience in hematology/oncology required.
  • Bachelor degree in Nursing preferred.
