Rehabilitation Specialist needed for full-time, temp to perm position in PG County. This position is office based with some fieldwork and begins immediately. The social worker will work with those with serious and persistent mental health issues.
Responsibilities:
Advises the President/CEO and Clinical Supervisor regarding development and implementation of rehabilitation services.
Supervises the direct staff regarding clinical concerns. Educates the direct services staff on how to provide rehabilitation services that correlate to the goals identified on the Individual Treatment Plan (person centered).
Able to trouble-shoot potential problems in client services and assist the Rehabilitation Coordinator in identifying resources and interventions including crisis intervention strategies.
Develops and assures that rehabilitation services meet the needs of the individuals served by the program.
Oversees all treatment plans and request for authorization for services.
Conducts (with appropriate staff) client assessments and reviews related treatment plans and transition plans. Review outcomes.
Assist in the transition planning discharge/termination process of clients as needed.
Conducts program specific client groups related to the rehabilitation goals.
Provides administrative oversight for residential and supportive housing programs, reports all findings/issues to the Clinical Supervisor and CEO.
Participates in administrative meetings and other official meetings as assigned. Acts as a liaison to community agencies.
Reviews client confidential records to ensure consistency with COMAR/H.I.P.P.A. regulations and timely documentation. Conducts peer-review of the client confidential records. 12. Rotates on the "on call" pager system.
