Job Summary

The Tribal Tobacco Prevention Program Coordinator will present tobacco prevention information, resources and evidence-based cessation programs to the CTUIR. All duties and responsibilities will be performed in accordance with the Yellowhawk Mission and Vision statements.

Essential Job Functions:

  1. Initiate and maintain community partnerships with all levels of organizational, Tribal and governmental units to assure wide based support of tobacco initiatives.
  2. Leads the development of health education and training programs for specific age and target groups.
  3. Coordinate promotion of program activities in schools, neighborhoods and workplaces.
  4. Utilize well developed oral communication and presentation skills for public speaking and face-to-face interactions.
  5. Develop expertise in evidence based or best practice tobacco prevention and cessation protocols.
  6. Develop and implement programming to meet objectives set within the TPEP grant.
  7. Create and manage the TPEP budget.
  8. Facilitate completion and submission of grants, renewals and reports to support program position and activities
  9. Coordinate with NPAIHB/TPEP and Yellowhawk Public Health Department to move the program forward each biennium.
  10. Review and interpret relevant tobacco data to prioritize activities.
  11. Create and maintain social media campaign in coordination with YTHC-PR personnel.
  12. Assist in the design, production, and dissemination of collateral marketing materials (letters, brochures, and flyers)
  13. Work closely with Education and other departments to provide culturally appropriate programs, materials and tobacco prevention activities for Tribal families.
  14. Coordinate and integrate services into Pharmacy, Medical, Public Health, Behavioral Health, Dental and Diabetes Prevention.
  15. Maintain, track and analyze patient data to determine the appropriate interventions for unique patient populations and work with management to develop more effective referral pathways based on analysis findings.
  16. Attend meetings and training as scheduled by NPAIHB/TPEP and Yellowhawk.
  17. Will utilize GSA vehicle for travel.
  18. Provide feedback to Supervisors, Medical Providers, clinicians and administrators on community priorities, needs and barriers on a regular basis to help catalyze the development of projects and needed community programs.
  19. Act as point person for event set up (securing space, transporting materials, ordering food, etc.)
  20. Develop and maintain positive relationships with the community, coworkers, supervisor, health professionals and clients.
  21. Attend clinic huddles and meetings as appropriate to market programs, communicate progress, and gather input/feedback for improvement.
  22. Track, maintain, and analyze patient data to determine the appropriate interventions for unique patient populations and work with management to develop more effective referral pathways based on analysis findings.
  23. Performs other related and incidental duties as needed or assigned.
  24. May need to work weekends, evenings, outside regular clinic hours or outside at outreach events.
  25. Other duties as assigned.
 
Position Requirements

Minimum Qualifications:

  1. Associates degree in any field.
  2.  Must demonstrate one (1) year of community health or population health experience.
  3. Must have First Aid/CPR within 30 days of employment.
  4. Demonstrated experience with Microsoft Office programs and/or other relevant software to meet the needs of the program.
  5. Must be detail-oriented, well-organized and able to multi-task while accurately completing activities related to position.
  6. Excellent communication skills both written and orally with the ability to speak before groups.
  7. Must possess a valid driver’s license and maintain insurance requirements to operate General Services Administration vehicles.
  8. Must submit to and pass a reference and criminal background check.
  9. Must submit to and pass a pre-employment drug and alcohol screening.
  10. Must provide proof of COVID-19 vaccination.

Preferred Qualifications:

  1. One (1) year experience working with the public in the public health education field.
  2. Tobacco Treatment Specialist (TTS) certification highly preferred.
  3. Motivational Interviewing (MI) certification highly preferred.
  4. Previous experience with policy development preferred
  5. Experience working with a grant, grant objectives, budgets and reporting preferred.
  6. Organized data management skills preferred.
