Kinship Navigator Case Manager

Looking for a full-time Kinship Case Manager (minimum of 35 hours)

 

Job Summary:

You will provide provide support services to grandparents, relatives, children, and other caregivers. These services include information and referral, case management, Kinship Legal Guardianship, and Wraparound services.

 

What You Will Do:

  • Evaluate and assess current and immediate needs of kinship families relating to families goals and needs at time of first initial request and on an on-going basis
  • Conduct home visits, collect documentation, and complete written assessment
  • Establish a trusting relationship with families through phone contact
  • Provide to families information and referral to accessible services
  • Engage and provide support to families through the information and referral process
  • Guide caregivers through the application process, including conducting an initial hierarchical needs assessment and providing additional information for referral to appropriate services
  • Be a liaison between caregivers, the program's contractor, and appropriate service providers
  • Promote advocacy skills, empowering clients through appropriate program and service education
  • Provide linkage referrals
  • Complete documentation within the program's allotted timeframe, as set by Program Manager and Program deliverables

 

What We Need to See:

  • Associate Degree degree in social work or other behavioral health discipline from an accredited college or university OR you may substitute additional experience as indicated on a year-for-year basis with thirty (30) semester hour credits being equal to one (1) year of experience. 
  • 1+ year work experience working with families, children and adolescent populations
  • A valid driver's license and must be insurable by agency's automobile insurance
