Job Summary

Tribal Navigator is responsible to assist Yellowhawk Seniors or disabled patients in navigating through and applying for State and Federal programs. (I.e. Medicare, Aging & People with Disabilities (APD). Works closely in collaboration with the State of Oregon, APD, CTUIR Department of Children and Families Services (DCFS), Community Health Department to identify senior needs and disabled. All duties and responsibilities will be performed in accordance with Yellowhawk’s mission and vision statement.

Essential Job Functions:

  1. Assist and track patients needing to apply for Medicare and/or APD.
  2. Responsible to promote strong coordination between APD, Community Health and DCFS.
  3. Responsible for outreach and education to patients that might be eligible for Medicare A, B & D plans.
  4. Responsible to assist and coordinate with APD for eligible clients/patients.
  5. Assist CTUIR members with enrolling in Medicare Part B and Medicare D.
  6. Responsible for all submitted applications through Medicare.gov.
  7. Responsible to keep current with all mandated trainings at the State and Federal levels and maintain all required certifications.
  8. Responsible to keep abreast of changes with Centers of Medicare & Medicaid (CMS) legislation, eligibility, benefits, etc.
  9. Responsible to assist patients with submitting required documentation for Medicare and/or APD.
  10. Provides regular reports to supervisor regarding intake and submitted applications.
  11. Researches the RPMS system for eligible beneficiaries to assist them with coverage for Medicare and/ or APD services.
  12. Other duties as assigned.
 
Position Requirements

Minimum Qualifications

  1. High school diploma or equivalent.
  2. Demonstrate basic knowledge of Medicare guidelines.
  3. Demonstrated ability to handle detail-oriented work with consistent follow through.
  4. Must possess a valid driver’s license and maintain insurance requirements to operate General Services Administration vehicles.
  5. Must submit to and pass a reference and criminal background check.
  6. Must submit to and pass a pre-employment drug and alcohol screening.
  7. Must submit proof of COVID-19 vaccination.

Preferred Qualifications

  1. Associate’s degree in social, behavioral or health science.
  2. Related experience assisting Elders or Disabled individuals.
  3. Work experience in human services (i.e. work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing)
  4. Extensive knowledge of Medicare guidelines.
