Case Manager Adoption House (PT)

Looking for a Part Time Case Manager for our Adoption House Program (20 hours)

 

Job Summary:
You will provide supervised visitation for parents and their children who have been placed in foster care to decrease the amount of time children spend in out-of-home placement and move families toward permanent placement process, either with their biological family or in an adoptive home.

 

What You Will Do:

  • Guide families through the reunification and adoption process, including participating in ongoing meeting with the biological parents and with the foster parent/caregiver and child
  • Provide weekly supervised visitation sessions, offer parent support and psycho-education, identify goals, and develop service plans
  • Conduct ongoing risk assessments, refer and coordinate services, and monitor compliance of the service plan and participation in services
  • Document progress in an accurate and objective manner and provide feedback regarding family interactions and additional services to ultimately reach permanency goals
  • Establish a trusting relationship with families through initiating and maintaining regular contact
  • Engage caregivers, support them through the process, promote advocacy skills, and provide linkage referrals
  • Maintain positive relationships will all collateral service providers and community partners, including Division of Child Protection and Permanency case workers, substance abuse counselors, therapists, foster parents, and extended family members
  • Transport children to and from visitation
  • Attend monthly Case Planning Team Meetings

 

What We Need to See:

  • A Bachelor's degree in Social Work or other behavioral health discipline from an accredited college or university
  • 2 years’ experience working with children and adolescent populations
  • Case management experience and documented knowledge of childhood development and mental health issue preferred
  • A valid driver's license and must be insurable by agency's automobile insurance
