Oversees the admission process within the Nursing department to ensure that each newly admitted resident is admitted into the EMR appropriately, which will include: medications, admission orders, treatment orders and all other aspects of care needed for the admission process. Completes all required admission assessments for admission. Responsibilities: Responsible for new admissions on all households. Ensures all medications are entered accurately into the EMR and faxed to pharmacy timely. Ensures efficiency and continuity of resident care. Functions as a Supervisor for clinical and administrative functions which includes leading RNs, LPNs, CNAs, and CMTs. Promotes interdepartmental communication. Ensures proper medication and treatment management. Conducts walking rounds to ensure the delivery of quality care for residents. Maintains and promotes high resident care standards. Responsible for a wide variety of nursing administrative duties. Monitors job performance of nursing staff and provides corrective action as needed. Promotes a team environment through effective communication and employee recognition. Participates in the on-call rotation of nurse management. Participates in the initial base line-care plans for all new admissions and all other meetings as necessary. Conducts follow-up phone calls with families after admission and otherwise necessary. Conducts audits as assigned by the Director of Nursing or designee. Works the floor as needed to ensure appropriate staffing levels are maintained.   Position Requirements Licensed Practical Nurse or Registered Nurse with active license in Kentucky. Minimum of (2) years of clinical experience in a health care setting. Minimum of (2) years of experience in a long-term care/short term rehab facility. Prior 1-year supervisory experience as a nurse supervisor, unit manager, or ADON. Must be capable of maintaining regular attendance. Clinical assessment skills at the level necessary to meet the job requirements. Basic working knowledge of Microsoft Office Suite. Experience with electronic medical records in long term care or short term-term rehab facility. Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long term care and short term rehabilitation.
