Job Summary

The Community Coordinator/Home Visitor (CC/HV) of the Yellowhawk Indigenous Project Linking Actions for Unmet Needs in Children's Health (ILAUNCH) grant will be responsible for providing direct outreach to the CTUIR community, family participants, providers, departments, and organizations involved in direct services, as well as community leaders and advocates. Additionally, the CC/HV will provide home visits to families using the evidence-based curriculums Family Spirit and Conscious Discipline. The CC/HV should have strong ties to the CTUIR community, have the ability to establish positive relationships, and engage community members. The CC/HV will be involved in the creation of a doula program during the 5th and final year of ILAUNCH. Traditional Health Worker - Doula training via DONA International and mentorship to be completed within 9 months of employment, dependent on training availability.

The purpose of this program is to promote the wellness of Tribal children from birth through eight years of age by addressing the physical, social, emotional, cognitive and behavioral aspects of their development. The goal of ILAUNCH is for children to be thriving in safe, supportive environments, and entering school ready to learn and succeed. This program will foster culturally-responsive models to support and promote mental health and wellness and reduce the impacts of trauma, including historical trauma and adversity, in American Indian/Alaska Native communities through a multigenerational and public health approach. All duties and responsibilities will be performed in accordance with the Yellowhawk Mission and Vision statement.

Essential Job Functions:

  1. Working under the direction of the Public Health Director and Grant Manager- plan, facilitate, and engage the community through several avenues of outreach that promote early-childhood interventions, activities and support healthy families.
  2. Communicate and collaborate closely with parent partner, project team, Yellowhawk Behavioral Health team, contracted OBGYN providers, and health promotion team.
  3. Serve as a helpful link between Behavioral Health services and Public Health services to create a more integrated approach for client success.
  4. Perform home visits to assess patient and family healthcare and social needs to promote positive parenting and healthy living.
  5. Understand grant objectives and strategies and aid in the collection and maintenance of grant data and reporting requirements.
  6. Complete doula training program and be active in the creation of policies, procedures, and early program implementation strategies.
  7. Advertise, organize, and lead/co-lead group sessions such as parenting classes.
  8. Assist in providing quality health promotion activities to the CTUIR community.
  9. Provide service link and referral resources to assist families with their healthcare and social needs.
  10. Collaborate closely with Yellowhawk departments, CTUIR entities, and partner agencies to ensure streamlined, quality services for youth and families.
  11. Other duties as assigned.
 
Position Requirements

Minimum Qualifications:

  1. Associate’s Degree in Social Work, Early Childhood Education, Psychology or related field

OR

  1. High School Diploma and 5 years relevant work experience.
  2. Demonstrated experience working with youth and families in Tribal communities.
  3. General understanding of trauma-informed approaches to working with women, children and families.
  4. Demonstrated ability to work effectively in a team setting with diverse membership
  5. Maintain lifestyle consistent with behavioral health role modeling of wellness.
  6. Maintain confidentiality in accordance with the Yellowhawk Tribal Health Center Policy Manual and the Health Insurance Portability and Accountability Act (HIPAA).
  7. Must possess or receive CPR and First Aid certification within ninety (90) days of employment.
  8. Must complete Traditional Health Worker – Doula training via DONA International and register on the Oregon Health Authority Birth Doula State Registry within 9 months of employment. (Dependent upon training availability.)
  9. Represents Yellowhawk with a respected and viable presence in the community.
  10. Must possess a valid driver’s license and maintain insurance requirements to operate General Services Administration vehicles.
  11. Must submit to and pass a reference and criminal background check.
  12. Must submit to and pass a pre-employment drug and alcohol screening.
  13. Must submit proof of COVID-19 vaccination.

Preferred Qualifications:

  1. Bachelor’s Degree in Social Work, Early Childhood Education, Public Health, or related field.
