Valley Community Services Board serves as the single point of entry into publicly funded mental health, developmental, and substance use disorder services for these localities.

Description

Valley Community Services Board is seeking a dynamic individual to join our Assertive Community Treatment Team. The ACT Case Manager provides intensive case management, rehabilitation, and supportive services to individuals experiencing serious mental illness. The incumbent will be working with a population of adults who may be dually diagnosed with chronic mental illness and substance use issue. Services are provided in accordance with the ACT model of treatment. Services are provided in multiple settings, including in the home and in the community.

Essential Functions

  • Supportive counseling, relationship skill-building, psychiatric rehabilitation counseling, symptom management, housing support, vocational readiness.
  • Routine community and home visits to assist consumers as defined by their individual treatment plan.
  • Crisis intervention, including during scheduled on-call times.
  • Meets requirements for documentation of client service delivery and maintains records in accordingly.
  • Uses person-centered treatment planning which reflects consumer strengths and preferences, as well as barriers to successful recovery and community living.
  • Assists individuals to maintaining independent living.
  • Provides individual, group and family rehabilitative and support counseling.
  • Assists in scheduling for and transportation to psychiatric, medical, medical disability determination, housing/landlord, and community support services appointments and meetings.
  • Provides emergency/crisis intervention and coordinates for hospital prescreening.
  • Assists and participates in all hospital diversion, hospital in-reach, and hospital discharge and reintegration services.
  • Assists in the provision of medical services, such as delivering and monitoring medications.  Delivers medications and oversees client self-administration of medications.
  • Provides clinical and program consultation to individuals, organizations, agencies, groups, etc. that are involved with VCSB consumers.

 

Knowledge, Skills, and Abilities

 

Knowledge of:

  • The ACT model of care
  • Biopsychosocial assessment
  • Human personality, growth, and development
  • MH & Co-Occurring Disabilities
  • Supportive counseling techniques, crisis intervention, psychosocial rehabilitation local and state community resources.
  • Skills pertaining to:
  • Evidence-based counseling techniques and theories of counseling
  • Completing diagnostic and assessment evaluations of current or potential ACT clients.
  • Mental status assessment, treatment planning, intervention implementation and evaluation of clients needs.
  • Providing emergency/crisis intervention services.

 

Ability to:

  • Work effectively with clients with SMI-Co-Occurring Disorders.
  • Effectively implement interagency coordination of services for the target client population
  • Exercise sound judgement
  • Demonstrates initiative
  • Maintain confidentiality and maintain ethical standards as be practice dictates
  • To work independently as well as within a team.
 
Position Requirements

Requirements

  • Bachelor's or master’s degree in human services or related field.
  • 1 Year direct clinical experience with mentally ill population
  • Valid VA driver’s license with an acceptable driving record
  • MUST be QMHP or QMHP-T eligible.
